龙胆泻肝汤加减治疗宫颈炎合并HPV感染临床观察

摘　　要：目的探究对脾虚湿毒蕴结型宫颈炎合并人乳头瘤病毒(HPV)感染患者实施龙胆泻肝汤加减治疗的效果。方法随机将2020年10月—2021年10月青岛西海岸新区人民医院收治的84例宫颈炎合并HPV感染患者分为对照组(42例,常规疗法)、试验组(42例,常规疗法联合龙胆泻肝汤加减治疗)。对比2组患者总有效率、HPV转阴率、阴道清洁度、炎性因子水平、HPV-DNA转阴时间、宫颈柱状上皮异位愈合时间。结果治疗后,试验组总有效率90.48%(38/42)高于对照组的66.67%(28/42),HPV转阴率33.33%(14/42)高于对照组的11.90%(5/42),差异均有统计学意义(P<0.05);试验组HPV-DNA转阴时间、宫颈柱状上皮异位愈合时间短于对照组,差异均有统计学意义(P<0.05);试验组阴道清洁度、炎性因子水平明显较对照组改善,差异均有统计学意义(P<0.05)。结论对脾虚湿毒蕴结型宫颈炎合并HPV感染患者实施龙胆泻肝汤加减治疗可显著增强效果、促进HPV转阴,且有助于减轻炎症反应,改善阴道清洁度。Objective To explore the effect of modified Longdan Xiegan decoction on patients with cervicitis complicated by human papillomavirus(HPV)infection of spleen deficiency and dampness accumulation type.Methods From October 2020 to October 2021,84 patients with cervicitis complicated with HPV infection in Qingdao Xihaian District People’s Hospital were randomly divided into control group(42 cases were treated with conventional therapy)and experimental group(42 cases were treated with conventional therapy and Longdan Xiegan decoction).The total effective rate,HPV negative conversion rate,vaginal cleanliness,inflammatory fac⁃tor level,HPV-DNA negative conversion time and ectopic healing time of cervical columnar epithelium were compared between the two groups.Results The total effective rate of experimental group was 90.48%(38/42),which was higher than that of control group(66.67%,28/42),and the negative HPV conversion rate of 33.33%(14/42),which was higher than that of control group(11.90%,5/42),and the difference was statistical significance(P<0.05).The negative transition time of HPV-DNA and ectopic healing time of cervical columnar epithelium in experimental groups were shorter than those in control group,and the differences were statistically significant(P<0.05).After treatment,the levels of vaginal cleanliness and inflammatory factors in experimental group were significantly improved compared with control group,and the differences were statistically significant(P<0.05).Conclusion Modified Longdan Xiegan decoc⁃tion in patients with cervicitis complicated with HPV infection of spleen deficiency and dampness accumulation type can significantly enhance the curative effect,promote HPV negative conversion,and help reduce inflammation and improve vaginal cleanliness.
