摘  要:目的:探讨中青年原发性高血压患者单核细胞/高密度脂蛋白比值(MHR)与臂踝动脉脉搏波传导速度(baPWV)及踝肱指数(ABI)的相关性。方法:选择我院住院患者216例,其中高血压156例(高血压组),非高血压60例(非高血压组)。采集及分析一般临床资料,比较各组间MHR、baPWV及ABI的差异,并分析MHR与baPWV和ABI之间的相关性。结果:与非高血压组比较,高血压组MHR和baPWV显著升高,ABI显著降低(P均=0.001)。与高血压低中危组比较,高危和极高危组中MHR、baPWV均显著升高、ABI均显著降低,且极高危组ABI显著低于高危组,P<0.05或<0.01。Pearson相关分析显示高血压患者MHR与baPWV呈显著正相关(r=0.794,P=0.001),MHR与ABI呈显著负相关(r=-0.696,P=0.001)。多元线性回归分析显示,MHR是baPWV的正向影响因素(B=3536.335,P=0.001),是ABI的负向影响因素(B=-0.356,P=0.001)。结论:高血压患者MHR与baPWV、ABI密切相关,联合检测可能有助于识别早期动脉硬化病变。Objective:To investigate the correlation among monocyte/high density lipoprotein ratio(MHR),brachial-ankle pulse wave velocity(baPWV)and ankle-brachial index(ABI)in young and middle-aged patients with essential hypertension(EH).Methods:A total of 216 inpatients from our hospital were selected,including 156 EH cases(EH group)and 60 non-EH cases(non-EH group).General clinical data were collected and analyzed,MHR,baPWV and ABI were compared between two groups,and correlation among above indexes was analyzed.Results:Compared with non-EH group,there were significant rise in MHR and baPWV,and significant reduction in ABI in EH group,P=0.001 all.Compared with low-medium risk EH group,there were significant rise in MHR and baPWV,and significant reduction in ABI in high and extremely high risk group,and ABI of extremely high risk group was significantly lower than that of high risk group,P<0.05 or<0.01.Pearson correlation analysis indicated that MHR was significant positively correlated with baPWV(r=0.794,P=0.001),and significant inversely correlated with ABI(r=-0.696,P=0.001)in EH patients.Multivariate linear regression analysis indicated that MHR was a positive influencing factor for baPWV(B=3536.335,P=0.001),and a negative influencing factor for ABI(B=-0.356,P=0.001).Conclusion:MHR is closely related to baPWV and ABI in hypertensive patients.Combined detection may help to identify early arteriosclerotic lesion.
