摘  要:目的:探讨心脏骤停(CA)后CMPK2/NLRP3轴对炎症反应的发生及参附注射液的调节机制。方法:使用随机数字法,随机选取20只大鼠作为空白对照组(BC组),另外将待进行制备模型的大鼠随机分为3组,假手术组(SO组)、正常复苏组(NR组)、参附组(SF组),每组20只。空白对照组不进行任何操作;假手术组不进行心脏骤停,每日注射生理盐水;正常复苏组在心脏骤停及复苏后每日注射生理盐水;参附组在心脏骤停时开始注射参附注射液,并在复苏后每日继续注射参附注射液。观察复苏后24 h及72 h各组心率(HR)、平均动脉压(MAP)、心尖组织HE染色情况,并使用qRT-PCR技术检测各组CMPK2、NLRP3、caspase-1、IL-1β相关mRNA表达情况。结果:24 h时NR组较SO组和SF组的MAP降低(P<0.05)。HE染色可见24 h时NR组及SF组心肌细胞部分破裂,炎性细胞浸润,72 h时,NR组及SF组细胞水肿较24 h明显减轻,细胞间隙仍可见部分炎性细胞存在。qRT-PCR发现,复苏后24 h, NR组、SF组较BC组和SO组的CMPK2、NLRP3、caspase-1、IL-1β相关mRNA表达均明显升高(P<0.05);复苏后72 h, NR组、SF组的NLRP3、caspase-1、IL-1β相关mRNA表达均仍较BC组和SO组高(P<0.05)。结论:CA发生后,CMPK2/NLRP3通路可被激活,并引起一系列炎症反应,使用参附注射液可抑制CMPK2/NLRP3的激活程度,减少炎症反应。Objective:To investigate the mechanisms of regulating CMPK2/NLRP3 axis by Shenfu injection on myocardial cell inflammatory response after cardiac arrest(CA).Method:Through random number method,20 rats were randomly selected as the blank control group(BC group).In addition,the rats to be used as models were randomly divided into three groups:sham surgery group(SS group),normal resuscitation group(NR group),and Shenfu group(SF group),with 20 rats in each group.The BC group did not undergo any procedures.The SS group did not undergo cardiac arrest but received daily injections of physiological saline.The NR group received daily injections of physiological saline after cardiac arrest and resuscitation.The SF group was initially injected with Shenfu injection during cardiac arrest and continued to receive Shenfu injection daily after resuscitation.The heart rate(HR),mean arterial pressure(MAP),and apical tissue HE staining in each group were observed,and qRT PCR technology was used to detect CMPK2,NLRP3,caspase-1,and IL-1βrelated mRNA expression in each group.Result:At 24th hour,the MAP of NR decreased compared to those of SS and SF groups(P<0.05).HE staining showed partial rupture of myocardial cells in NR and SF groups and infiltration of inflammatory cells at 24th hour;at 72th hour,cell edema of NR and SF groups was significantly reduced compared to that at 24th hour,and some inflammatory cells were still visible in the intercellular space.QRT-PCR found that 24 hours after resuscitation,the expressions of CMPK2,NLRP3,caspase-1,and IL-1βrelated mRNAs in NR and SF groups were increased more than those in BC and SS groups(P<0.05).72 hours after recovery,the expressions of NLRP3,caspase-1,and IL-1βrelated mRNA in NR and SF groups were still higher than those in BC and SS groups(P<0.05).Conclusion:After the occurrence of CA,the CMPK2/NLRP3 pathway can be activated and cause a series of inflammatory reactions.Shenfu injection can inhibit the activation of CMPK2/NLRP3 and reduce the occurrence of inflammatory reactions
