雷火灸联合负压封闭引流治疗高位肛周脓肿的效果分析

摘　　要：目的:探究雷火灸联合负压封闭引流技术(VSD)治疗高位肛周脓肿的效果。方法:回顾性选取南京医科大学附属淮安第一医院在2020年1月—2022年12月间收治的110例高位肛周脓肿患者纳入本次研究,采用随机数字表法将其分为两组,每组55例,对照组行VSD治疗,观察组行雷火灸联合VSD治疗。比较两组临床疗效、炎症水平、疼痛应激反应、血清指标、恢复情况及并发症。结果:观察组疗效高于对照组(P<0.05)。治疗后,两组白细胞计数(WBC)、白细胞介素1β(IL-1β)、C反应蛋白(CRP)、去甲肾上腺素(NE)、皮质醇(Cor)、视觉模拟评分法(VAS)水平低于治疗前(P<0.05),且观察组低于对照组(P<0.05)。治疗后,两组血管内皮生长因子(VEGF)、表皮细胞生长因子(EGF)水平高于治疗前(P<0.05),且观察组高于对照组(P<0.05)。与对照组相比,观察组创面渗液评分下降,住院及创面愈合时间缩短(P<0.05)。两组并发症差异无统计学意义(P>0.05)。结论:雷火灸联合VSD治疗用于高位肛周脓肿临床治疗,可减轻炎症及疼痛应激,促进创面愈合,且治疗安全性较高,从而可有效提高临床疗效。Objective:To explore the effect of thunder fire moxibustion combined with vacuum sealing drainage(VSD)technique on the treatment of high perianal abscess.Methods:We retrospectively included in this study 110 patients with high perianal abscess admitted to Huaian First Affiliated Hospital of Nanjing Medical University between January 2020 and December 2022.The patients were divided into two groups according to a random number table,with 55 patients in each group.The control group received VSD treatment,while the observation group was treated with thunder-fire moxibustion combined with VSD.The clinical efficacy,inflammation level,pain stress response,serum indexes,recovery and complications were compared between the two groups.Results:The therapeutic effect in the observation group was higher than that of the control group(P<0.05).After the treatment,the levels of white blood cell count(WBC),interleukin-1β(IL-1β),C-reactive protein(CRP),norepinephrine(NE),cortisol(Cor),and visual analogue scale(VAS)in both groups were reduced,however,the reduction levels in the observation group was more than those in the control group(P<0.05).After the treatment,the levels of vascular endothelial growth factor(VEGF)and epidermal growth factor(EGF)in both groups were increased,while the increased level was more in observation group than in the control group(P<0.05).Compared with the control group,the observation group experienced decrease in wound exudation score and shorter hospitalization and wound healing time(P<0.05).There was no statistically significant difference in complications between the two groups(P>0.05).Conclusion:Thunder fire moxibustion combined with VSD for the clinical treatment of high perianal abscess can reduce inflammation and pain stress,and promote wound healing,and has high treatment safety,thereby effectively improving the clinical efficacy.
