单管双腔H型引流管在慢性硬膜下血肿钻孔引流术中的应用效果分析

摘　　要：目的:分析单管双腔H型引流管在慢性硬膜下血肿钻孔引流术中的应用效果。方法:选取2017年1月—2021年12月射阳县人民医院收治的慢性硬膜下血肿患者147例作为研究对象,以随机数字表法分为对照组(n=73)和观察组(n=74),两组均行钻孔置管引流术,对照组采用传统硅胶引流管,观察组采用改良单管双腔H型引流管。比较两组治疗效果。结果:观察组治疗总有效率高于对照组,术后第1天血肿残余量少于对照组,置管时间和住院时间短于对照组,并发症总发生率、血肿复发率低于对照组,差异有统计学意义(P<0.05)。术后1周,两组改良Rankin评分均低于术前,且观察组低于对照组,差异有统计学意义(P<0.05)。结论:单管双腔H型引流管在慢性硬膜下血肿钻孔引流术中的应用效果显著,能够减少血肿残余量,降低并发症发生率,改善患者神经功能,缩短置管时间,促进康复,预防血肿复发。Objective:To analyze the application effect of single-cannula double-lumen H-shaped drainage tube in chronic subdural hematoma drilling and drainage surgery.Methods:A total of 147 patients with chronic subdural hematoma admitted to Sheyang County People's Hospital from January 2017 to December 2021 were selected as the study subjects.They were divided into control group(n=73)and observation group(n=74)by random number table method.Both groups underwent drilling catheterization and drainage surgery,the control group used traditional silicone drainage tube,and the observation group used modified single-cannula double-lumen H-shaped drainage tube.The treatment effects of the two groups were compared.Results:The total effective rate of treatment in the observation group was higher than that in the control group,the residual amount of hematoma on 1 day after surgery in the observation group was less than that in the control group,the catheterization time and hospital stay in the observation group were shorter than those in the control group,and the total incidence of complications and hematoma recurrence rate in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).At 1 week after surgery,the modified Rankin score in two groups was lower than that before surgery,and the observation group was lower than the control group,and the difference was statistically significant(P<0.05).Conclusion:The application effect of single-cannula double-lumen H-shaped drainage tube in chronic subdural hematoma drilling and drainage is remarkable,which can reduce the residual amount of hematoma,decrease the incidence of complications,improve patients'neurological function,shorten the catheterization insertion time,promote recovery,and prevent hematoma recurrence.
