双源CT血管成像技术诊断主动脉夹层的价值分析

摘　　要：目的:探讨双源CT血管成像技术诊断主动脉夹层的价值。方法:选取2019年1月—2022年1月内蒙古乌兰察布市第三医院收治的80例疑似主动脉夹层患者作为研究对象,均进行数字减影血管造影以及双源CT血管成像技术检查。分析数字减影血管造影以及双源CT血管成像技术检查的诊断结果,比较真腔、假腔的CT强化值及不同重建技术对破口、内膜瓣、真假腔的清晰显示率。结果:80例疑似患者中,经数字减影血管造影检查确诊35例,双源CT血管成像技术检查结果与数字减影血管造影检查一致。真腔CT强化值高于假腔,差异有统计学意义(P=0.001)。多平面重建(MPR)、曲面重建(CPR)对主动脉夹层破口显示率较高,分别为85.71%、82.86%,容积再现(VR)、表现遮盖显示(SSD)较低,最大强度投影(MIP)不显示;MPR、VR、CPR、SSD内膜瓣显示率为100.00%,MIP不显示;MPR、CPR、VA、SSD对真假腔显示率为100.00%,MIP为77.14%。MPR、CPR、VR、SSD对破口、内膜瓣、真假腔的显示率均高于MIP,差异有统计学意义(P<0.05)。结论:双源CT血管成像技术在主动脉夹层诊断中的应用价值显著,能够准确辨别真假腔;在重建技术中,MPR、CPR对主动脉夹层的显示效果较好。Objective:To investigate the value of dual-source CT angiography in diagnosing aortic dissection.Methods:A total of 80 patients with suspected aortic dissection who were admitted to the Third Hospital of Ulanqab City from January 2019 to January 2022 were selected as the study subjects.All patients received digital subtraction angiography and dual-source CT angiography.The diagnostic results of digital subtraction angiography and dual-source CT angiography were analyzed,and the CT enhancement values of true lumen and false lumen and the clear display rate of fractures,intimal flaps,true lumen and false lumen by different reconstruction techniques were compared.Results:Among the 80 suspected patients,35 cases were confirmed by digital subtraction angiography,and the results of dual-source CT angiography were consistent with those of digital subtraction angiography.The CT enhancement value of true lumen was higher than that of false lumen,and the difference was statistically significant(P=0.001).Multiplanar reconstruction(MPR)and curved planar reformation(CPR)showed high rates of aortic dissection fractures,accounting for 85.71%and 82.86%,respectively,while volume rendering(VR)and surface shaded display(SSD)showed lower rate,and maximum intensity projection(MIP)did not display.The display rates of MPR,VR,CPR,SSD for intimal flaps were 100.00%,and MIP did not display;The display rates of MPR,CPR,VA,SSD for true lumen and false lumen were 100.00%,and that of MIP was 77.14%.The display rates of MPR,CPR,VR and SSD for fractures,intimal flaps,true lumen and false lumen were higher than that of MIP,and the difference was statistically significant(P<0.05).Conclusion:The application value of dual-source CT angiography technology in the diagnosis of aortic dissection is significant,which can accurately distinguish true lumen from false lumen.In reconstruction technology,MPR and CPR have a good effect on displaying aortic dissection.
