摘  要:目的 基于潜在类别分析不同纤维蛋白单体(FM)水平与妊娠糖尿病(GDM)发生的关系。方法 选择2020年3月至2022年3月于衡水市妇幼保健院就诊筛查确诊符合早期GDM产妇(162例)为研究对象。依据妊娠期间是否发生GDM分为GDM组(80例)和非GDM组(82例),收集产妇一般资料对比分析。影响GDM发生的危险因素采用多因素Logistic回归分析,比较GDM发生高风险组和低风险组间GDM发生影响因素分布特征的差异采用潜在类别分析法。结果 与非GDM组相比,GDM组产妇GDM发生率更高(P<0.05)。多因素Logistic回归分析,结果显示,糖尿病家族史、孕早期C-反应蛋白(CRP)≥8 mg/L、孕早期血红蛋白(Hb)≥130 g/L、D-二聚体(D-D)≥1.47μg/mL、纤维蛋白原(FIB)≥7 g/L、心肌酶谱≥250 U/L以及FM≥6μg/mL均为GDM发生的独立危险因素(P<0.05)。不同FM水平产妇的GDM发生概率差异显著(P<0.05)。聚类分析结果显示,GDM发生高风险组的GDM发生概率(60.34%)显著高于低风险组(21.74%)(P<0.05)。潜在类别分类结果显示,与GDM发生低风险组相比,高风险组中“危险因素较多型分布”占比更高(P<0.05)。结论 不同FM水平下的GDM发生率存在差异,且GDM发生高风险组和低风险组GDM影响因素的分布特征也存在差异。Objective The relationship between different fibrin monomer(FM)levels and the occurrence of gestational diabetes mellitus(GDM)was analyzed based on the latent class.Methods From March 2020 to March 2022,162 women with early GDM diagnosed by screening in Hengshui Maternal and Child Care Service Centre were selected as the research objects.According to whether GDM occurred during pregnancy,they were divided into GDM group(80 cases)and non-GDM group(82 cases).Multivariate Logistic regression analysis was used to analyze the risk factors affecting the occurrence of GDM.Latent class analysis was used to compare the distribution characteristics of influencing factors of GDM between high-risk group and low-risk group.Results Compared with non-GDM group,the incidence of GDM in GDM group was higher(P<0.05).Multivariate Logistic regression analysis showed that family history of diabetes,C-reactive protein(CRP)≥8 mg/L,hemoglobin(Hb)≥130 g/L,D-dimer(D-D)≥1.47μg/mL,fibrinogen(FIB)≥7 g/L,myocardial enzyme spectrum≥250 U/L and FM≥6μg/mL were independent risk factors for GDM(P<0.05).There was a significant difference in the incidence of GDM among pregnant women with different FM levels(P<0.05).The results of cluster analysis showed that the probability of GDM in the high-risk group(60.34%)was significantly higher than that in the low-risk group(21.74%)(P<0.05).The results of latent class classification showed that compared with the low-risk group of GDM,the proportion of“more risk factors distribution"in the high-risk group was higher(P<0.05).Conclusion The incidence of GDM under different FM levels is different,and the distribution characteristics of GDM influencing factors in high-risk group and low-risk group are also different.
