妊娠期孕妇PAI-1基因启动区4G5G位点多态性与高血压对早产的协同作用

摘　　要：目的 探究妊娠期孕妇纤溶酶原激活物抑制物-1(PAI-1)基因启动区4G5G位点多态性与妊娠高血压(HDP)对早产的协同作用。方法 连续性纳入2018年12月至2022年12月于南京市第二医院分娩的无血缘关系的早产妊娠期孕妇72例为早产组,另选取同期于南京市第二医院正常分娩的妊娠期孕妇60例为对照组。筛选妊娠期孕妇早产的影响因素,采用受试者操作特征(ROC)曲线、校准曲线评价模型效益,分析PAI-1基因启动区4G5G位点多态性与HDP的交互作用对早产的影响。结果 年龄、HDP、早产史、PAI-1基因型、胎膜早破、羊水过多是妊娠期孕妇早产的独立危险因素(P<0.05),ROC曲线和校准曲线显示该模型区分度和校准度良好。PAI-1基因型(4G/5G)、(4G/4G)与HDP均存在正交互作用(γ>1)。结论 携带PAI-1 4G/4G基因型与HDP的交互作用可促进妊娠期早产的发生。Objective To explore the synergistic effect of maternal plasminogen activator inhibitor-1(PAI-I)gene promoter 4G5G polymorphism and hypertension on preterm birth.Methods 72 unrelated preterm pregnant women who gave birth in the Second Hospital of Nanjing from December 2018 to December 2022 were consecutively included as the preterm group,and another 60 pregnant women who gave birth normally in the Second Hospital of Nanjing during the same period were selected as the control group.The influencing factors of premature delivery in pregnant women during pregnancy were analyzed.Receiver operating characteristics(ROC)curve and calibration curve were used to evaluate the model benefit,and the interaction between 4G5G site polymorphism of PAI-1 gene promoter region and HDP on preterm birth was analyzed.Results Age,HDP,preterm birth history,PAl-1 genotype,premature rupture of membranes and hyperamniotic fluid were independent risk factors for preterm birth in pregnant women(P<0.05).ROC curve and calibration curve show that the model is well differentiated and calibrated.PAI-1 genotype(4G/5G),(4G/4G)had positive interaction with HDP(y>1).Conclusion The interaction between PAI-14G/4G genotype and HDP can promote the occurrence of preterm labor during pregnancy.
