摘　　要：目的探究实时剪切波弹性成像(SWE)对冈上肌腱损伤的诊断价值,比较其与二维超声的诊断价值差异。方法选取行关节镜手术的76例患者,根据关节镜结果将患者分为无撕裂组、部分撕裂组和全层撕裂组,术前行二维超声及SWE比较3组患者患侧和健侧的弹性模量值,绘制ROC曲线分析SWE对冈上肌腱损伤的诊断价值。结果(1)与无撕裂组相比,撕裂组的弹性模量值显著降低;(2)3组患侧的弹性模量值均显著低于健侧(P<0.05);(3)SWE诊断冈上肌全层撕裂的AUC为0.962,稍高于二维超声和磁共振,差异无统计学意义(P>0.05),SWE诊断冈上肌腱部分撕裂的AUC为0.878,显著高于二维超声及磁共振(P<0.05)。结论SWE在诊断冈上肌损伤类型及程度具有应用价值,能够辅助二维超声为临床诊断治疗肩袖损伤提供有价值的信息。Objective To explore the diagnostic value of shear ware elastography(SWE)in the supraspinatus tendon injury,and compare its diagnostic value with that of two-dimensional ultrasound.Methods A total of 76 patients who underwent arthroscopic surgery in our hospital were included and divided into three groups:the no tear group,the partial tear group,and the full layer tear group based on the results of arthroscopic surgery.Before surgery,two-dimensional ultrasound and SWE were performed to compare the elastic modulus values of the affected and healthy sides of the three groups.ROC curves were drawn to analyze the diagnostic value of SWE for the supraspinatus tendon injury.Results(1)Compared with the no tear group,the Young's modulus value of the tearing group was significantly reduced;(2)The elastic modulus values on the affected side of the three groups were significantly lower than those on the healthy side(P0.05);(3)The AUC of SWE diagnosing full supraspinatus tendon full layer tear was 0.962,slightly higher than that of the two-dimensional ultrasound and MRI,but with no statistically significant difference(P0.05).The AUC of SWE diagnosis for supraspinatus tendon partial tear was 0.878,significantly higher than that of two-dimensional ultrasound and MRI(P0.05).Conclusions SWE has practical value in determing the type and degree of supraspinatus tendon injury,and can assist with two-dimensional ultrasound in providing valuable information for diagnosis and treatment of rotator cuff injury.
