摘　　要：目的探讨角化棘皮瘤(KA)的临床特点及超声表现。方法收集经病理证实的KA患者共43例,结合其临床特点,分析并总结该病的超声表现。结果43例KA多见于老年男性患者(67.4%),均为单发(100.0%),好发于四肢(55.8%)及头颈部(27.9%),基底部位于真皮层(100.0%),瘤体多向皮肤表面生长(95.4%),质地较韧(67.4%),表面可伴有溃疡(44.2%)。KA超声表现为低回声团(100.0%),形态呈椭圆形(62.8%)、扁平状(16.3%)和不规则(20.9%),内回声均匀(39.5%)或不均匀(60.5%),边界清晰(44.2%)或不清晰(58.8%),部分伴有后方回声衰减(53.5%),大多数以真皮层供血(80.0%)为主,呈树枝状血流信号,Adler分级0~Ⅲ级。结论KA的临床特点及超声表现具有一定的特征性,需注意与其他皮肤良恶性肿瘤相鉴别。Objective To explore the clinical features and ultrasonic manifestations of keratoacanthoma(KA)in the skin.Methods A total of 43 KA patients presented to our hospital were analyzed and the ultrasonic manifestations combined with their clinical features were summarized.Results A total of 43 solitary KA were more commonly located in the limbs(55.8%)than the head or neck(27.9%)in elderly male patients(67.4%),which grew from the dermis(100.0%)to the surface of the skin(95.4%)with tough texture(67.4%)and surface ulcer(44.2%).The ultrasound manifestations of KA were hypoechoic(100.0%),which were oval(62.8%),flat(16.3%),or irregular(20.9%)in shapes,uniform(39.5%)or uneven(60.5%)in the interior,well-defined(44.2%)or ill-defined(58.8%)in the boundary with posterior echo attenuation(53.5%).Most KA showed dendritic blood flow signals supplied by the dermis(80.0%)whose level scales of Adler were 0 toⅢ.Conclusions The clinical features and ultrasound manifestations of KA have certain characteristics.However,attention still should be paid to distinguish from other benign and malignant tumors in the skin.
