叙事疗法联合康复操对乳腺癌化疗患者负性情绪、睡眠及生活质量的影响

摘　　要：目的 探讨叙事疗法联合康复操在乳腺癌化疗患者中的应用效果。方法 选择2018年7月-2021年7月医院收治的乳腺癌化疗患者120例作为研究对象,按照组间基本特征具有可比性的原则分为对照组和观察组,每组60例。对照组患者采取常规护理,观察组患者在此基础上采用叙事疗法联合康复操干预。比较两组患者干预前后Zung焦虑自评量表(SAS)和抑郁自评量表(SDS)、匹兹堡睡眠质量指数量表(PSQI)、自我护理能力及生活质量评分。结果 护理干预前,两组患者SAS、SDS、PSQI、自我护理能力及生活质量评分的比较,差异无统计学意义(P>0.05)。实施叙事疗法联合康复操后,观察组患者SDS、SAS评分及PSQI各维度评分均低于对照组(P<0.05);自我护理能力各维度评分、生存质量各维度及总均分均高于对照组(P<0.05)。结论 叙事疗法联合康复操应用于乳腺癌化疗患者中,可明显减轻患者负性情绪,提高患者睡眠质量及自我护理能力,进而提高患者生活质量。Objective To explore the application effect of narrative therapy combined with rehabilitation exercises in breast cancer chemotherapy patients.Methods 120 cases of breast cancer chemotherapy patients admitted to the hospital from July 2018 to July 2021 were selected as the study object,they were divided into the control group and the observation group according to the principle that the basic characteristics between the groups were comparable,with 60 cases in each group.The patients in the control group adopted conventional care,and the patients in the observation group adopted narrative therapy combined with rehabilitation exercise intervention on this basis.Self-rating Anxiety Scale(SAS)and Self-rating Depression Scale(SDS),Pittsburgh Sleep Quality Index(PSQI),self-care ability and quality of life scores before and after the intervention were compared between the two groups.Results Before the nursing intervention,the comparison of SAS,SDS,PSQI,self-care ability and quality of life scores between the two groups was not statistically signifi cant(P>0.05).After the implementation of narrative therapy combined with rehabilitation exercises,the SDS,SAS scores and PSQI scores of the dimensions of the patients in the observation group were lower than those of the control group(P<0.05).The scores of the dimensions of self-care ability,the dimensions of the quality of life and the total mean scores were higher than those of the control group(P<0.05).Conclusion Narrative therapy combined with rehabilitation exercises applied to breast cancer chemotherapy patients could signifi cantly reduce patients'negative emotions,improve patients'sleep quality and self-care ability,improve patients'quality of life.
