三种不同手势对女性PICC置管患者血流动力学的影响

摘　　要：目的 对比观察PICC置管的女性肿瘤患者,分别在手掌张开、普通握拳与握大鱼际肌三种手势下,置管侧贵要静脉、肱静脉、头静脉的血流速度,探索有效提高PICC置管后上肢静脉血流速度的握拳方式,为PICC置管后预防导管相关静脉血栓最佳握拳方式提供参考依据。方法 选取2022年9-12月医院PICC置管女性患者17例为研究对象,患者分别进行手掌张开、普通握拳与握大鱼际肌三种手部姿势,使用彩超分别检测PICC置管患者三种手部姿势下贵要静脉、肱静脉、头静脉的血流峰值流速。结果 测试结果显示,患者手掌张开、普通握拳、握大鱼际三种手势状态下,贵要静脉、肱静脉和头静脉的血流速度差异有统计学意义(P<0.05),其中握大鱼际时血流速度最快,手掌张开时最慢。结论 女性肿瘤患者PICC置管后进行握大鱼际肌手势能提高患者PICC置管侧贵要静脉、肱静脉、头静脉的血流速度。Objective To compare and observe the blood flow velocity of the noble vein,brachial vein and cephalic vein on the side of catheter placement in female oncology patients with PICC catheterisation under three gestures,namely,open palm,normal fi st clenched and clenched big fi ssure muscle,and to explore the fi st-clenched gesture that can effectively improve the venous flow velocity of the upper limb after PICC catheterisation,so as to provide a reference basis for the optimal fi st-clenched gesture in preventing catheter-related venous thrombosis after PICC catheterisation.Methods 17 female patients with PICC catheterisation in the hospital from September to December 2022 were selected as the study subjects,and the patients were subjected to three hand postures,namely,open palm,ordinary fi st and large interosseous muscle,and the peak flow velocities of the veins of the vital vein,the brachial vein and the cephalic vein were detected using colour ultrasound in the three hand postures of the patients with PICC catheterisation,respectively.Results The research results showed that the difference in blood flow velocity of the vital vein,brachial vein and cephalic vein in the three hand positions of patients with open palms,normal clenched fi sts,and clenched big fi ssure muscles was statistically signifi cant(P<0.05),with the fastest blood flow velocity in clenched big fi ssure muscles,and the slowest in the open palms.Conclusion Female oncology patients with PICC placement after performing the grip of the large fi ssure muscle gesture can improve the blood flow velocity of the noble vein,brachial vein and cephalic vein on the side of the patient's PICC placement.
