人工流产后继发不孕症的多因素分析

摘　　要：目的分析人工流产后继发不孕症的相关影响因素。方法回顾性分析2019年1月至2020年1月本院收治的100例人工流产患者的临床资料,统计人工流产后继发不孕症的发生情况,采用单因素及多因素Logistic回归分析影响人工流产后继发不孕症的危险因素。结果100例患者中,继发不孕症25例,发生率为25.00%;成功受孕75例,成功受孕率为75.00%。继发不孕症患者与成功受孕患者年龄、受教育年限、月收入水平、初潮年龄、手术方式、月经不调及性生活频率比较差异无统计学意义;继发不孕症患者与成功受孕患者宫腔粘连、慢性输卵管炎、卵巢功能障碍、人工流产史、初次性生活年龄及性伴侣数比较差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,宫腔粘连、慢性输卵管炎、卵巢功能障碍、人工流产史>2次及初次性生活年龄≤20岁是影响人工流产后继发不孕症的独立危险因素(OR>1,P<0.05)。结论影响人工流产后继发不孕症的危险因素为宫腔粘连、慢性输卵管炎、卵巢功能障碍、人工流产史>2次及初次性生活年龄≤20岁,临床需重视并及时采取相关干预措施,以改善人工流产后的妊娠结局。Objective To analyze the related influencing factors of secondary infertility after artificial abortion.Methods The clinical data of 100 patients with artificial abortion admitted to our hospital from January 2019 to January 2020 were retrospectively analyzed,the occurrence of sec-ondary infertility after artificial abortion was statistically analyzed,and univariate and multivariate Logistic regression analysis were used to analyze the risk factors of secondary infertility after artificial abortion.Results Among the 100 patients,25 cases were secondary infertility,and the inci-dence of 25.00%;75 cases were successfully pregnant,and the successful pregnancy rate was 75.00%.There were no statistically significant differ-ences in age,years of education,monthly income,age of menarche,operation method,menstrual irregularity and sexual frequency between the pa-tients with secondary infertility and patients with successful pregnancy;while there were statistically significant differences in intrauterine adhesions,chronic salpingitis,ovarian dysfunction,history of induced abortion,age of first sexual life and number of sexual partners between the patients with secondary infertility and patients with successful pregnancy(P<0.05).Multivariate Logistic regression analysis showed that intrauterine adhesions,chronic salpingitis,ovarian dysfunction,history of induced abortion>2 times and age of first sexual life≤20 years old were independent risk factors for secondary infertility after induced abortion(OR>1,P<0.05).Conclusion The risk factors of secondary infertility after artificial abortion are in-trauterine adhesions,chronic salpingitis,ovarian dysfunction,history of artificial abortion>2 times,and age of first sexual life≤20 years old,clini-cal attention should be paid and relevant intervention measures should be taken in time to improve the pregnancy outcomes after artificial abortion.
