低剂量克拉霉素在鼻内镜下鼻中隔矫正术联合鼻窦开放术治疗慢性鼻-鼻窦炎中的应用效果及对患者ECP、TIgE水平的影响

摘　　要：目的分析低剂量克拉霉素在鼻内镜下鼻中隔矫正术联合鼻窦开放术治疗慢性鼻-鼻窦炎中应用效果及对患者血清中嗜酸性粒细胞阳离子蛋白(ECP)、总免疫球蛋白E(TIgE)水平的影响。方法选取2019年3月至2021年3月本院接收的90例慢性鼻-鼻窦炎患者作为研究对象,采用随机数字表法分为参照组与研究组,各45例。参照组行鼻内镜下鼻中隔矫正手术联合鼻窦开放手术治疗,研究组在参照组治疗基础上给予低剂量克拉霉素治疗,比较两组治疗效果和术后2、4、8周血清中ECP、TIgE水平。结果研究组术后治疗有效率为93.33%,高于参照组的77.78%,差异有统计学意义(P<0.05)。两组术后2、4、8周ECP、TIgE水平组间、时间、交互比较差异有统计学意义(P<0.05);组内比较:两组术后不同时间点的ECP、TIgE水平均低于前一时间点,两两比较差异有统计学意义(P<0.05);组间比较:术后2、4、8周,研究组ECP、TIgE水平低于参照组,差异有统计学意义(P<0.05)。结论鼻内镜下鼻中隔矫正术及鼻窦开放术治疗慢性鼻-鼻窦炎中采用低剂量克拉霉素可提高术后疗效,减轻炎症反应,值得临床推广应用。Objective To analyze the application effect of low-dose clarithromycin in the treatment of chronic rhinosinusitis by nasal endoscopic septum correction combined with open sinus operation and its effect on the level of eosinophilic cationic protein(ECP)and total immunoglobulin E(TIgE)in serum.Methods 90 patients with chronic rhinosinusitis admitted to our hospital from March 2019 to March 2021 were selected as the re-search subjects,and they were divided into reference group and study group based on the random number table method,with 45 cases in each group.The reference group underwent nasal endoscopic septum correction combined with open sinus operation,the study group received low-dose clar-ithromycin treatment on the basis of the reference group,the postoperative treatment effects and serum levels of ECP and TIGE at 2,4 and 8 weeks after surgery were compared between the two groups.Results The effective rate of postoperative treatment in the study group was 93.33%,which was higher than the 77.78%in the reference group,the difference was statistically significant(P<0.05).There were significant differences in ECP and TIgE level inter-group,time and interaction between the two groups at 2,4 and 8 weeks after surgery(P<0.05).Comparison within groups:the ECP and TIgE levels of the two groups at different time points after surgery were lower than the previous time point,and the pairwise comparison had statistical significance(P<0.05).Comparison between groups:ECP and TIgE levels in the study group were lower than those in the reference group at 2,4 and 8 weeks after operation,and the difference was statistically significant(P<0.05).Conclusion Low-dose clarithromycin can im-prove postoperative efficacy and reduce inflammatory reactions in the treatment of chronic rhinosinusitis using nasal endoscopic septum correction combined with open sinus operation,which is worthy of clinical promotion and application.
