不同手术时机对结石嵌顿性胆囊炎患者预后的影响研究

摘　　要：目的探讨不同手术时机对结石嵌顿性胆囊炎患者预后的影响。方法选取2020年1月至2021年6月包头市核工业208大队包头博华医院收治的100例结石嵌顿性胆囊炎患者作为研究对象,随机分为对照组(发病72 h后接受手术治疗)与研究组(发病72 h内接受手术治疗),每组50例。两组均行腹腔镜四孔法进行手术,比较两组围手术期指标、并发症发生情况及术中转开腹率。结果研究组术中出血量少于对照组,手术时间、术后肛门排气时间、术后疼痛时间、住院时间均短于对照组,差异有统计学意义(P<0.05)。研究组并发症发生率低于对照组,差异有统计学意义(P<0.05)。研究组术中转开腹率低于对照组,差异有统计学意义(P<0.05)。结论在结石嵌顿性胆囊炎病程72 h内接受手术治疗能降低术后并发症发生率,保证患者治疗效果,缩短术后恢复时间。Objective To explore the effect of different operation timing on the prognosis of patients with incarcerated stone cholecystitis.Methods A total of 100 patients with incarcerated stone cholecystitis admitted to Baotou Bohua Hospital,Nuclear Industry 208 Brigade from Janu-ary 2020 to June 2021 were selected as the research subjects,and they were randomly divided into the control group(received surgery 72 h after on-set)and the study group(received surgery within 72 h after onset),with 50 cases in each group.Both groups were given laparoscopic four-hole oper-ation,the perioperative indexes,the incidence of complications and the rate of conversion to laparotomy were compared between the two groups.Results The blood loss in the study group was less than that in the control group,and the operation time,postoperative anal exhaust time,postopera-tive pain time and hospitalization time were shorter than the control group,and the differences were statistically significant(P<0.05).The incidence of complications in the study group was lower than the control group,and the differences were statistically significant(P<0.05).The conversion rate in the study group to laparotomy was lower than the control group,and the difference was statistically significant(P<0.05).Conclusion Surgical treatment within 72 h of incarcerated stone cholecystitis can reduce the incidence of postoperative complications and ensure the therapeutic effect of patients,reduce postoperative recovery time.
