婴幼儿腹泻病原微生物检测及血清干扰素-γ白细胞介素-6转化生长因子-β的变化分析

摘  要:目的研究婴幼儿腹泻病原微生物检测及血清干扰素-γ(IFN-γ)、白细胞介素-6(IL-6)、转化生长因子-β(TGF-β)的变化情况。方法收集2021年1月至2022年12月新郑华信民生医院300例腹泻婴幼儿(观察组)及健康体检儿童100名(对照组)为研究对象,并将观察组患儿依据病情严重程度分为轻症组(140例)和重症组(160例)。统计观察组患儿的病原微生物情况;测定观察组和对照组、轻症组和重症组、细菌性和病毒性感染婴幼儿的血清IFN-γ、IL-6、TGF-β水平;并以临床综合检查结果为金标准,分析血清IFN-γ、IL-6、TGF-β的诊断效能。结果300例婴幼儿腹泻中以轮状病毒(42.0%)为主,其次为杯状病毒(17.3%)、大肠埃希菌(15.7%)。观察组的血清IFN-γ、IL-6、TGF-β水平高于对照组(P<0.05)。联合指标对婴幼儿腹泻的诊断敏感度、特异度、准确度均高于单一指标(IFN-γ、IL-6、TGF-β),差异有统计学意义(P<0.05)。轻症组、重症组的性别、年龄、病程比较差异无统计学意义(P>0.05),且重症组的血清IFN-γ、IL-6、TGF-β水平均高于轻症组(P<0.05)。细菌性感染婴幼儿腹泻的血清IFN-γ、IL-6、TGF-β水平高于病毒性感染婴幼儿(P<0.05)。结论婴幼儿腹泻主要由轮状病毒感染为主,可利用血清IFN-γ、IL-6、TGF-β水平对本病及其严重程度进行判定,值得推广。Objective To study the pathogenic microorganisms′diarrhea detection and serum interferon-gamma(IFN-gamma),interleukin 6(IL-6),transforming growth factor beta(TGF-beta)changes.Methods From January 2021 to December 2022 for collecting time,300 cases of infant diarrhea(observation group)and the physical examination of 100 children(control group)as the research object,and the observation group of children basis on the illness severity was divided into mild group(n=140)and severe group(n=160).Statistical observation group of pathogenic microorganisms of children;Determination of two groups(observation group and control group,mild group and severe group,bacterial and viral infections)in infants of serum IFN-gamma,IL-6,TGF-beta level;And as the gold standard by clinical comprehensive examination results,analysis of serum levels of IFN-gamma,IL-6,TGF-beta diagnostic efficiency.Results Three hundred cases of infant diarrhea were given priority to rotavirus(42.0%),followed by goblet virus(17.3%),e.coli(15.7%).Observation group serum IFN-gamma,IL-6,and TGF-beta levels were higher than the control group(P<0.05).The joint index to the diagnostic sensitivity,specificity and accuracy of infant diarrhea was higher than that of the single index(IFN-gamma,IL-6,TGF-beta),the difference was statistically significant(P<0.05).Mild group,the comparison of the gender,age,and course of severe groups(P>0.05),and the severe group serum IFN-gamma,IL-6,and TGF-beta levels were higher than in the mild group(P<0.05).Serum levels of IFN-gamma bacterial infection in infants and young children diarrhea,IL-6,and TGF-beta levels higher than the viral infection(P<0.05).Conclusion Infant diarrhea is mainly composed of rotavirus infection is given priority to,can use serum IFN-gamma,IL-6,and TGF-beta level to determine the disease and its severity,worthy of promotion.
