慢性中耳炎患者鼓室成形术后第1天骨传导听阈变化及影响因素分析

摘　　要：目的探讨慢性中耳炎患者鼓室成形术后第1天骨传导听阈变化及影响因素。方法选取2021年5月至2022年10月在成都中医药大学附属医院耳鼻喉科经鼓室成形术治疗的246例慢性中耳炎患者为研究对象,根据不同慢性中耳炎类型(单纯型、骨疡型、胆脂瘤型)和鼓室成形术式将其分为4组:Ⅰ组(Ⅰ型鼓室成形术,n=61)、Ⅱ组(上鼓室开放+Ⅱ型鼓室成形术,n=73)、Ⅲ组(上鼓室开放+Ⅲ型鼓室成形术,n=41)、Ⅳ组(上鼓室和乳突开放+Ⅲ型鼓室成形术,n=71),分析患者术后第1天骨传导听阈变化与鼓室成形术式及慢性中耳炎分型的关系。结果术后第1天4组骨传导听阈变化幅度均<15 dB,且Ⅰ组骨传导听阈减小最明显。4组骨传导听阈及骨传导听阈变化幅度(<-5 dB、-5~0 dB、0 dB、0~5 dB、>5 dB)比较,差异有统计学意义(P<0.05);术后第1天3种不同慢性中耳炎类型的骨传导听阈改变比较,差异有统计学意义(P<0.05)。结论慢性中耳炎患者鼓室成形术后第1天无严重感音神经性听觉障碍发生,但鼓室成形术式、慢性中耳炎分型对术后第1天骨传导听阈变化有影响。Objective To investigate the changes of bone conduction hearing threshold in patients with chronic otitis media on the next day after tympanoplasty and its influencing factors.Methods A total of 246 inpatients with chronic otitis media in the Otolaryngology Department of Hospital of Chengdu University of Traditional Chinese Medicine from May 2021 to October 2022 were selected as the study objects.According to different types of chronic otitis media(simple type,bone ulcer type,cholesteatoma type)and tympanoplasty,they were divided into 4 groups:GroupⅠ(typeⅠtympanoplasty,n=61),GroupⅡ(attic opening+typeⅡtympanoplasty,n=73),GroupⅢ(attic opening+typeⅢtympanoplasty,n=41),and GroupⅣ(attic opening+mastoid opening+typeⅢtympanoplasty,n=71).The relationship between the changes of bone conduction hearing threshold and tympanoplasty type and chronic otitis media type was analyzed.Results On the next day after tympanoplasty,the change of bone conduction hearing threshold in all 4 groups was less than 15 dB,and the decrease of bone conduction hearing threshold was the most obvious in Group I.There were significant differences in bone conduction hearing threshold and its amplitude of change(<-5 dB,-5-0 dB,0 dB,0-5 dB,>5 dB)among the 4 groups(P<0.05).There was significant difference in the changes of bone conduction hearing threshold on the next day after tympanoplasty among the three different types of chronic otitis media(P<0.05).Conclusion No serious sensorineural hearing impairment occurs in patients with chronic otitis media on the next day after tympanoplasty.However,the type of tympanoplasty and chronic otitis media could affect the change of bone conduction hearing threshold on the next day after tympanoplasty.
