改良小丑医疗服务在烧伤儿童换药中的应用

摘　　要：目的探讨在烧伤患儿换药中应用改良小丑医疗服务的效果。方法将年龄1~14岁的门诊就诊烧伤患儿随机分成两组,其中对照组43例,试验组50例。对照组患儿在换药中接受常规护理,试验组患儿除接受常规护理外,在换药前、中、后接受改良小丑医疗服务。比较两组患儿在换药前、中、后的疼痛评分、换药依从性以及家属和医务人员对改良小丑医疗服务的满意度。结果试验组患儿在换药中、后疼痛评分均低于对照组(P<0.05)。试验组患儿依从性较对照组高,差异有统计学意义(P<0.05)。家属和医务人员对改良小丑医疗服务均表示非常满意。结论改良小丑医疗服务能有效减轻患儿疼痛,提高患儿换药依从性,家属和医务人员满意度高。Objective To explore the effectiveness of modified medical clown service in burned children receiving dressing changes.Methods A total of 93 burned children aged 1 to 14 years old in the outpatient department of the First Affiliated Hospital of Chengdu Medical College from January 2021 to December 2022 were selected and randomly assigned to a control group(n=43)or an experimental group(n=50).When receiving dressing changes,the control group was given routine nursing,while the experimental group received the modified medical clown service on the basis of routine nursing before,during,and after dressing changes.Pain scores before,during,and after dressing changes,compliance with dressing changes,and satisfaction of family members and medical staff with the medical clown service were compared between the two groups.Results The pain scores during and after dressing changes in the experimental group were lower than those in the control group(P<0.05).The compliance with dressing changes in the experimental group was higher than that in the control group,and the difference was statistically significant(P<0.05).Family members and medical staff were very satisfied with the modified medical clown service.Conclusion The modified medical clown service can effectively alleviate pain in burned children,improve their compliance with dressing changes,and family members and medical staff are highly satisfied with it.
