20G Y型留置针引导下PICC穿刺术在新生儿置管中的应用

摘　　要：目的:探讨PICC穿刺术在新生儿置管中应用20G Y型留置针进行引导的效果。方法:选取2019年2月至2020年10月在本院采用PICC置管建立静脉通路的新生儿96例,将其按照随机数字表法分为观察组(n=48)与对照组(n=48),观察组采用20G Y型留置针引导下PICC穿刺术,对照组采用塞丁格穿刺针进行穿刺,观察并分析两组穿刺成功率、出血率、操作时间、管路留置时间、疼痛程度、并发症发生率及家属满意度。结果:观察组穿刺成功率明显高于对照组(P<0.05),出血率明显低于对照组(P<0.05),操作时间明显短于对照组(P<0.05),管路留置时间明显长于对照组(P<0.05),疼痛程度明显低于对照组(P<0.05),感染、堵管、静脉炎等并发症发生率明显低于对照组(P<0.05),家属满意度明显高于对照组(P<0.05)。结论:PICC穿刺术在新生儿置管中应用20G Y型留置针进行引导,可有效提高穿刺成功率,缩短操作时间,增加管路留置时间,降低新生儿疼痛程度,减少出血及并发症,提高家属满意度,值得推广应用。Objective:To investigate the effect of PICC puncture guided by 20G Y indent needle in neonatal catheterization.Methods:A total of 96 neonates who received PICC catheterization to establish venous access in our hospital from February 2019 to October 2020 were selected and divided into observation group(n=48)and control group(n=48)according to random number table method.PICC puncture guided by 20G Y indwelling needle was used in observation group,and Seidinger needle was used in control group.The success rate of puncture,bleeding rate,operation time,catheter indwelling time,pain level,Complication rate and family satisfaction of the two groups were observed and analyzed.Results:Puncture success rate of observation group was significantly higher than control group(P<0.05),bleeding rate was significantly lower than control group(P<0.05),operation time was significantly shorter than control group(P<0.05),pipeline indwelling time was significantly longer than control group(P<0.05),the pain level was significantly lower than that of the control group(P<0.05),infection,tube blockage,phlebitis and other complications were significantly lower than control group(P<0.05),the satisfaction of family members was significantly higher than that of the control group(P<0.05).Conclusion:PICC puncture is guided by a 20G Y-shaped indwelling needle in neonatal cannulation,which can effectively improve the success rate of puncture,shorten the operation time,increase the indwelling time of the pipeline,reduce the degree of neonatal pain,reduce bleeding and complications,and improve family members.Satisfaction,it is worth promoting and applying.
