证候用药禁忌的历史沿革、现代研究与思考

摘　　要：“证候用药禁忌”一词在现代文献中才被明确提出,但关于这一概念的论述却早已有之。“证候用药禁忌”在历史上的不同时期有多种称谓方式,如“药禁”“禁忌”“药害”等。文章总结“证候用药禁忌”概念内涵的演变,内容的丰富、完善,证候用药禁忌术语的确立与规范,梳理了证候用药禁忌的发展脉络和现代研究,提出证候用药禁忌研究中存在的问题,进行深入思考,提出了相应的建议。建议中药证候用药禁忌的研究应根据古代文献的记载、临床实践经验的积累、临床不良反应的研究以及毒理实验研究的结果,进行系统整理,建立安全性评价体系,同时不断完善证候禁忌药的范畴以及单味中药的证候禁忌范围,加强中成药和注射剂的禁忌证候研究、体质用药禁忌的研究,为临床用药更加安全合理提供科学依据。The term‘contraindications to symptomatic medication’was clearly put forward in modern literature,but the discussion on this concept has already existed for a long time.In different periods of history,there are many ways to address the‘contraindications to symptomatic medication’,such as‘drug prohibition’‘taboo’‘drug harm’etc..This article summarizes the evolution of the connotation of the concept of‘syndrome drug taboo’,the enrichment and improvement of the content,and the establishment and standardization of the terminology of syndrome drug taboo,combs the development context and modern research of syndrome drug taboos,puts forward the problems existing in the research of syndrome drug taboos,carries out indepth thinking,and puts forward corresponding suggestions.We suggest that the study of traditional Chinese medicine syndrome based contraindications to symptomatic medication should be based on the records of ancient literature,the accumulation of clinical experience,the study of clinical adverse reactions,and the results of toxicological experiments.We should systematically sort out,establish a safety evaluation system,constantly improve the scope of syndrome contraindication drugs and the scope of contraindications to symptomatic medication of single Chinese medicine,and strengthen the research on the contraindications of Chinese patent medicine and constitutional medicine.To provide scientific basis for more safe and reasonable clinical medication.
