基于“土得木而达”理论探讨子宫内膜息肉的临床证治

摘　　要：子宫内膜息肉是妇科临床常见病,中医学据其病本特点将其归于“癥瘕”病范畴,结合五行理论和现代妇女生理病理特点,认为其发生与肝脾两藏关系密切,基本病机为肝脾失调,气血失和,主要病理要素为痰湿、瘀血。“土得木而达”是《黄帝内经》中高度概括肝脾生克互用关系的理论,故文章基于此,探讨肝脾在子宫内膜息肉防治中的重要作用,提出息肉未成或金刃术后防复应以疏肝健脾为主,兼调气血,常用方为苍附导痰丸加减;息肉已成则以祛痰瘀为先,兼顾肝脾,常用方为桃红四物汤合二陈汤加减。Endometrial polyp is a common gynecological clinical disease.According to its characteristics,TCM classifies it into the category of‘Zheng Jia’disease.Combined with the theory of five elements and the physiological and pathological characteristics of modern women,it is believed that its occurrence is closely related to the liver and spleen,the basic pathogenesis of which is liver and spleen disorders,the disharmony of qi and blood,and the main pathological elements are phlegm,dampness and blood stasis.The theory of‘earth being dredged by wood’in the Huangdi Neijing highly generalizes the relationship between the liver and spleen in the prevention and treatment of endometrial polyps.Therefore,based on this,this paper discusses the important role of the liver and spleen in the prevention and treatment of endometrial polyps,and proposes that the prevention and treatment of polyps that are not formed or after the golden knife surgery should be based on soothing the liver and strengthening the spleen,and regulating the qi and blood.The commonly used formula is Cangfu Daotan Pill plus or minus;If the polyp has been formed,the phlegm and blood stasis should be removed first,and the liver and spleen should be taken into account.The common formula is Taohong Siwu Decoction and Erchen Decoction plus or minus.
