补肾活血法治疗糖尿病肾病Ⅲ、Ⅳ期患者临床疗效观察

摘　　要：目的:探讨补肾活血法对糖尿病肾病(DKD)患者的临床疗效和安全性。方法:于2020年6月—2021年8月多中心纳入217例DKDⅢ、Ⅳ期患者,随机分为对照组(108例)和试验组(109例)。对照组采取基础治疗,试验组在对照组基础上加用补肾活血中药,治疗3个月后比较两组患者临床总有效率、中医症状积分及理化指标,记录用药期间不良反应。结果:试验组患者治疗后的临床总有效率为63.9%(69/108),显著高于对照组的46.8%(51/109)(P<0.05)。两组患者治疗后中医症状积分均较治疗前显著下降(P<0.01)。治疗后试验组患者肌酐、尿素氮、尿蛋白和血糖水平均较治疗前显著下降(P<0.05,P<0.01),较对照组更加显著(P<0.01,P<0.05)。两组患者在治疗期间均未出现明显的不良反应。结论:补肾活血法治疗DKDⅢ、Ⅳ期患者临床疗效满意,能够有效改善患者临床症状、肾损伤指标及血糖水平,具有良好的安全性。Objective:To investigate the clinical efficacy and safety of Bushen Huoxue therapy in patients with diabetic kidney disease(DKD)stage Ⅲ and Ⅳ.Methods:A total of 217 patients with DKD stage Ⅲ and Ⅳ from multicenter from June 2020 to August 2021 were enrolled in this study.They were randomly assigned to either the control group(108 cases),receiving conventional basic treatment,or the treatment group(109 cases),receiving the same treatment as the control group plus traditional Chinese medicine(TCM)with Bushen Huoxue effects.After 3 months of treatment,clinical total effective rate,TCM symptom score,and physical and chemical indicators were compared between the two groups,and adverse reactions during medication were recorded.Results:The clinical total effective rate of the experimental group patients was 63.9%(69/108),which was higher than that of the control group at 46.8%(51/109)(P<0.05).Both groups of patients showed a significant decrease in TCM symptom scores after treatment compared to before treatment(P<0.01).After treatment,the experimental group patients had a greater improvement in creatinine,blood urea nitrogen,urinary protein,and blood glucose levels(P<0.05,P<0.01),and it was more obvious when compared to the control group(P<0.01,P<0.05).There were no significant adverse reactions observed in either group during the treatment period.Conclusion:The therapeutic effcacy of Bushen Huoxue in the treatment of DKD stage Ⅲ and Ⅳ is satisfactory.This approach effectively improves the clinical symptoms,renal injury indicators,and blood glucose levels of patients,demonstrating good safety.
