摘 要:目的 探析血清视黄醇结合蛋白(retinol binding protein,RBP)、β_(2)-微球蛋白(β_(2)-microglobulin,β_(2)-MG)、胱抑素C(cystatin C,CysC)水平联合检测对妊娠期糖尿病(gestational diabetes mellitus,GDM)待产妇早期肾损害的预测价值。方法 选择2021年7月—2022年6月莆田九十五医院接收的90例GDM待产妇作为研究对象,基于24 h尿蛋白排泄率(24 h urinary protein excretion rate,UAER)进行分组,分为单纯GDM组(n=43)、合并早期肾损伤组(n=47),同期选取29例健康孕妇。对比GDM待产妇及健康孕妇RBP、β_(2)-MG、CysC水平,比较不同UAER水平GDM待产妇RBP、β_(2)-MG、CysC水平,绘制ROC曲线,评估上述指标单独、联合检测效能(灵敏度与特异度)。结果 GDM待产妇RBP、β_(2)-MG、CysC水平高于健康孕妇,差异有统计学意义(P<0.05)。单纯GDM组RBP、β_(2)-MG、CysC水平低于合并早期肾损伤组,差异有统计学意义(P<0.05)。RBP、β_(2)-MG、CysC水平单独检测阳性检出率依次为72.34%、74.47%、65.92%,低于联合检测的93.62%,差异有统计学意义(P<0.05)。ROC曲线结果表明RBP、β_(2)-MG、CysC水平联合检测在GDM预测中的灵敏度、特异度均高于单一RBP、β_(2)-MG、CysC水平检测,差异有统计学意义(P<0.05)。结论 对GDM待产妇实施RBP、β_(2)-MG、CysC水平检测可预测是否合并早期肾损害,综合分析各项数据可为临床提供肾损伤相关信息。Objective To explore the prediction of early renal damage in expectant mothers with gestational diabetes mellitus(GDM) by combined detection of serum retinol-binding protein(RBP),β_(2)-microglobulin(β_(2)-MG),and cystatin C(CysC) levels value.Methods From July 2021 to June 2022,90 pregnant women with GDM received by Putian 95 Hospital were selected as the research objects.Based on 24 h urinary protein excretion rate(UAER),they were divided into simple GDM group(n=43) and combined with early renal injury group(n=47).At the same time,29 healthy pregnant women were selected.The levels of RBP,β_(2)-MG and CysC in GDM pregnant women and healthy pregnant women were compared.The levels of RBP,β_(2)-MG and CysC in GDM pregnant women with different UAER levels were compared.The ROC curve was drawn to evaluate the efficacy of single and combined detection of the above indicators(sensitivity and specificity).Results The levels of RBP,β_(2)-MG and CysC in pregnant women with GDM were higher than those in healthy pregnant women,and the differences were statistically significant(P<0.05).The levels of RBP,β_(2)-MG and CysC in the simple GDM group were lower than those in the early renal injury group,and the differences were statistically significant(P<0.05).The positive detection rates of RBP,β_(2)-MG and CysC levels in single test were 72.34%,74.47% and 65.92%,respectively,which were lower than 93.62% in combined test,and the differences were statistically significant(P<0.05).The results of ROC curve showed that the sensitivity and specificity of combined test of RBP,β_(2)-MG and CysC levels in GDM prediction were higher than those of single RBP,β_(2)-MG and CysC level test,and the difference was statistically significant(P<0.05).Conclusion The detection of RBP,β_(2)-MG and CysC levels in pregnant women with GDM can predict whether they are complicated with early renal damage,and comprehensive analysis of the data can provide clinical information about renal damage.
