老年2型糖尿病患者发生低血糖的影响因素分析

摘　　要：目的 探讨老年2型糖尿病患者低血糖的影响因素。方法 选取2021年3月—2022年3月在湖南省中西医结合医院内分泌老年病科住院的老年2型糖尿病患者81例为研究对象,采用回顾性分析方法根据住院期间是否发生低血糖分为两组,观察组37例、对照组44例,对采集的变量进行单因素分析,采用Logistic逐步回归分析方法分析低血糖的影响因素。结果 观察组患者使用促泌剂、睡前血糖≤6 mmol/L、内生肌酐清除率≤60 mL/min、ALT≥2倍正常值上限的例数多于对照组,按时进食的例数少于对照组,差异有统计学意义(P<0.05)。采用Logistic回归分析结果显示,睡前血糖≤6 mmol/L、使用促泌剂、ALT≥2倍正常值上限是老年2型糖尿病患者发生低血糖的危险因素(P<0.05),内生肌酐清除率≤60 mL/min和按时进食为老年2型糖尿病患者发生低血糖的保护因素(P<0.05)。结论 老年2型糖尿病患者发生低血糖的影响因素是多重的,可能主要与促泌剂的使用、睡前血糖水平、肝肾功能、是否按时进食密切相关。Objective To investigate the influencing factors of hypoglycemia in elderly patients with type 2 diabetes mellitus.Methods 81 elderly patients with type 2 diabetes hospitalized in the Endocrine Geriatrics Department of Hunan Integrated Hospital of Traditional Chinese and Western Medicine from March 2021 to March 2022 were selected as the research objects.Retrospective analysis method was used to divide them into two groups according to whether hypoglycemia occurred during hospitalization,37 cases in the observation group and 44 cases in the control group.Single factor analysis was conducted on the collected variables,and Logistic stepwise regression analysis was used to analyze the influencing factors of hypoglycemia.Results The number of patients in the observation group who used secretagogues,had preoperative blood glucose≤6 mmol/L,endogenous creatinine clearance rate≤60 mL/min,and ALT≥2 times the upper limit of normal values were higher than those in the control group,the number of patients who ate on time was lower than that in the control group,and the difference was statistically significant(P<0.05).Logistic regression analysis showed that bedtime blood glucose ≤ 6 mmol/L,use of secretagogues,and ALT ≥ 2 times the upper limit of normal were risk factors for hypoglycemia in elderly patients with type 2 diabetes(P<0.05).Endogenous creatinine clearance rate ≤ 60 mL/min and eating on time were protective factors for hypoglycemia in elderly patients with type 2diabetes(P<0.05).Conclusion There are multiple influencing factors for hypoglycemia in elderly patients with type 2diabetes,which may be closely related to the use of secretagogues,blood glucose levels before going to bed,liver and kidney function,and whether to eat on time.
