摘 要:目的 探究康柏西普玻璃体内注射治疗糖尿病性黄斑水肿(diabetic macular edema,DME)的疗效及对黄斑区视网膜微循环的影响。方法 选择2019年5月—2022年5月闽侯县医院收治的150例DME患者为研究对象,根据随机数表法分成观察组及对照组,各75例。观察组行康柏西普玻璃体内注射治疗,对照组行雷珠单抗玻璃体内注射治疗。比较两组患者治疗前后最佳矫正视力(best corrected visual acuity,BCVA)、黄斑中心凹视网膜厚度(central macular thickness,CMT)、视网膜浅层毛细血管丛(superficial capillary plexus,SCP)的血管密度、深层毛细血管(deep capillary plexus,DCP)的血管密度以及黄斑中心凹无血管区面积(foveal avascular zone,FAZ)。结果 治疗前,两组BCVA、CMT比较,差异无统计学意义(P>0.05);治疗后,观察组BCVA高于对照组,CMT低于对照组,差异有统计学意义(P<0.05)。治疗前后两组SCP/DCP血管密度及FAZ比较,差异无统计学意义(P>0.05)。结论 康柏西普玻璃体内注射治疗DME疗效确切,能改善患者视力,减轻黄斑水肿,且对黄斑区视网膜微循环无明显影响,值得临床应用。Objective To explore the efficacy of intravitreal injection of Conbercept in the treatment of diabetic macular edema(DME) and its effect on retinal microcirculation in the macular area.Methods A total of 150 DME patients admitted to Minhou County Hospital from May 2019 to May 2022 were selected as the research objects.They were divided into observation group and control group by random number table method,with 75 cases in each group.The observation group was treated with intravitreal injection of conbercept,and the control group was treated with intravitreal injection of ranibizumab.The best corrected visual acuity(BCVA),central macular thickness(CMT),vascular density of superficial capillary plexus(SCP),vascular density of deep capillary plexus(DCP) and foveal avascular zone(FAZ) were compared between the two groups before and after treatment.Results Before treatment,there was no statistically significant difference in BCVA and CMT between the two groups(P>0.05).After treatment,the BCVA of the observation group was higher than that of the control group,and the CMT was lower than that of the control group,the difference was statistically significant(P<0.05).There was no statistically significant difference in SCP/DCP vascular density and FAZ between the two groups before and after treatment(P>0.05).Conclusion Intravitreal injection of Conbercept is effective in treating DME.It can improve patients' vision and reduce macular edema without significant impact on retinal microcirculation in the macular area,which is worthy of clinical application.
