摘 要:目的 分析对糖尿病肾病患者实施高通量与低通量血液透析治疗的应用效果及对其炎症反应、氧化应激影响的差异。方法 选取2020年1月—2022年1月山西省汾阳医院肾病科收治的60例糖尿病肾病患者为研究对象,依照血液透析治疗的差异将患者分为A组(n=30)和B组(n=30)。A组实施高通量血液透析治疗,B组实施低通量血液透析治疗,治疗结束后对比两组患者的炎症反应、氧化应激、肾功能情况以及不良反应发生情况。结果 治疗后两组患者的各项肾功能指标、炎症反应指标以及氧化应激指标较治疗前均有改善,差异有统计学意义(P<0.05),其中A组的尿素氮、肌酐及血尿酸、白介素6、肿瘤坏死因子α、丙二醛水平均低于B组,差异有统计学意义(P<0.05);A组的过氧化物歧化酶与谷胱甘肽过氧化物酶水平高于B组,差异有统计学意义(P<0.05)。A组的不良反应发生率(10.00%)与B组(16.67%)相比,差异无统计学意义(P>0.05)。结论 在糖尿病肾病患者的血液透析治疗中,采取高通量的血液透析治疗相比于低通量的血液透析治疗能更显著地降低炎症反应及氧化应激程度,改善患者的肾功能,不会额外增加治疗中的不良反应。Objective To analyzed the differences in the application effects of high-flux and low-flux hemodialysis treatments on patients with diabetic nephropathy and their impact on patients' inflammatory response and oxidative stress.Methods 60 patients with diabetic nephropathy admitted to the Nephrology Department of Fenyang Hospital in Shanxi Province from January 2020 to January 2022 were selected as study subjects.According to the difference in hemodialysis treatment,the patients were divided into group A(n=30) and group B(n=30).Group A received high-flux hemodialysis treatment,and group B received low-flux hemodialysis treatment.After treatment,the inflammatory response,oxidative stress,renal function and occurrence of adverse reactions were compared between the two groups of patients.Results After treatment,various renal function indicators,inflammatory response indicators and oxidative stress indicators of the two groups of patients were improved compared with those before treatment,and the difference was statistically significant(P<0.05).Among them,the levels of urea nitrogen,creatinine,serum uric acid,interleukin 6,tumor necrosis factor α and malondialdehyde in group A were lower than those in group B,and the difference was statistically significant(P<0.05);the levels of superoxide dismutase and glutathione peroxidase in group A were higher than those in group B,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse reactions in group A(10.00%) and group B(16.67%)(P>0.05).Conclusion In the hemodialysis treatment of patients with diabetic nephropathy,high-flux hemodialysis treatment can significantly reduce the degree of inflammatory response and oxidative stress and actively improve the patient's renal function compared with low-flux hemodialysis treatment.It will not increase the incidence of adverse reactions during treatment.
