摘 要:60岁及以上老年人是流行性感冒(流感)和肺炎链球菌性疾病(pneumococcal disease,PD)的高危人群,接种流感病毒裂解疫苗(influenza vaccine,InfV)和肺炎链球菌疫苗(pneumococcal vaccine,PV)可有效保护老年人免于流感和PD感染或减轻相关症状,减少死亡。国内外多项研究报道老年人联合接种InfV和PV在肺炎发病率、流感感染率、住院率和死亡率等方面要优于单独接种。本文从老年人联合接种InfV和PV的必要性、安全性、免疫原性、接种效果以及成本效益等方面阐述老年人联合接种的可行性并进一步提出接种建议,为优化我国老年人InfV和PV的接种策略提供参考。InfV和PV在我国仍属于非免疫规划疫苗,我国老年人2种疫苗接种率均处于较低水平。现阶段,新型冠状病毒感染(coronavirus disease2019,COVID-19)大流行仍在持续,老年人COVID-19病例中合并感染流感病毒和/或肺炎链球菌将进一步加重疾病负担或死亡风险,联合接种InfV和PV为提高老年人疫苗依从性和接种率的有效策略。Adults aged 60 years or older are at high risk for influenza and pneumococcal disease(PD),and immunization with the influenza vaccine(InfV)and pneumococcal vaccine(PV)can effectively protect the elderly from infection,mitigate symptoms,and prevent death.Some studies have reported that co-administration of InfV and PV in older adults was better than separate administration in terms of pneumonia incidence,influenza infection,hospitalization rate,and mortality.In this paper,the feasibility of co-administration of InfV and PV in the elderly is discussed from the aspects of necessity,safety,immunogenicity,effectiveness and cost-benefit,and recommendations for vaccination are proposed to provide reference for optimizing the vaccination strategy of InfV and PV in the elderly in China.Neither InfV nor PV is covered in the immunization program in China,and the uptake of these two vaccines is at a low level among the elderly.At this stage,the coronavirus disease 2019(COVID-19)pandemic is still ongoing,and co-infection with influenza and/or pneumococci in elderly COVID-19 cases will further increase the burden of disease or the risk of death.Co-administration of InfV and PV may be an effective strategy to improve vaccine compliance and uptake in older adults.
