瑞芬太尼联合异丙酚用于甲状腺切除手术麻醉的临床分析

摘　　要：目的讨论瑞芬太尼联合异丙酚用于甲状腺切除手术麻醉的临床分析。方法入选100例甲状腺切除手术麻醉患者主要于2021年1月-2022年1月接受病情诊疗,将其随机分为对照组50例与研究组50例。对照组采取瑞芬太尼麻醉方案,研究组采取瑞芬太尼联合异丙酚麻醉方案。观察指标:各时段心率及平均动脉压、麻醉恢复时间指标、麻醉不良反应。结果麻醉前两组心率及平均动脉压指标无统计学意义(P>0.05);气管插管时、切皮时、分离颈前筋膜时、缝皮时研究组心率及平均动脉压指标比对照组更平稳,有统计学意义(P<0.05)。研究组自主呼吸恢复、意识恢复、清醒、拔管麻醉恢复时间指标均明显短于对照组,有统计学意义(P<0.05)。研究组麻醉不良反应总发生机率的4%明显低于对照组的22%,有统计学意义(P<0.05)。结论甲状腺切除手术麻醉中采取瑞芬太尼联合异丙酚方案利于稳定患者心率及血压水平,减少麻醉不良反应,促进术后麻醉恢复,临床价值显著。Objective To discuss the clinical analysis of remifentanil combined with propofol for anesthesia during thyroidectomy.Methods A total of 100 anesthesia patients undergoing thyroidectomy were enrolled from January 2021 to January 2022 and randomly divided into control group(n=50)and study group(n=50).The control group received remifentanil anesthesia program,the study group received remifentanil combined with propofol anesthesia program.Observation indicators:heart rate and average arterial pressure at each time period,anesthesia recovery time index,anesthesia adverse reactions.Results Before anesthesia,there were no significant differences in heart rate and mean arterial pressure between the two groups(P>0.05).Heart rate and mean arterial pressure in the study group were more stable than those in the control group during endotracheal intubation,skin resection,anterior cervical fascia separation and skin suture,with statistical significance(P<0.05).The recovery time indexes of spontaneous breathing,consciousness,wakefulness and extubation anesthesia in the study group were significantly shorter than those in the control group,with statistical significance(P<0.05).The incidence rate of adr in the study group(4%)was significantly lower than that in the control group(22%),with statistical significance(P<0.05).Conclusion Remifentanil combined with propofol during thyroidectomy anesthesia can stabilize the heart rate and blood pressure levels of patients,reduce adverse anesthetic reactions,and promote postoperative anesthesia recovery,with significant clinical value.
