摘　　要：目的探讨阶梯式压力治疗在婴幼儿深度烧伤后瘢痕防治中的应用效果。方法选取2019年12月至2020年12月在我院创面治疗中心治疗的86例深度烧伤患儿为研究对象,按随机数字表法将其分为对照组与试验组,每组43例。对照组给予常规抗瘢痕治疗,试验组在对照组基础上给予阶梯式压力治疗。比较两组的瘢痕严重程度、不良反应总发生率、家属满意度及依从性。结果治疗3、6、12个月后,试验组的温哥华瘢痕量表(VSS)评分低于对照组,差异具有统计学意义(P<0.05)。试验组创面愈合后3个月内的不良反应总发生率低于对照组,差异具有统计学意义(P<0.05);两组创面愈合后4~6个月、7~12个月的不良反应总发生率比较,差异无统计学意义(P>0.05)。治疗3、6、12个月后,试验组家属的满意度高于对照组,差异具有统计学意义(P<0.05)。试验组的遵医嘱治疗、按时复诊依从率高于对照组,差异具有统计学意义(P<0.05)。结论阶梯式压力治疗对婴幼儿深度烧伤后瘢痕具有较好的防治效果,可减轻瘢痕严重程度,降低不良反应发生率,提高患儿家庭满意度及患儿依从率,值得临床推广与应用。Objective To investigate the application effect of stepped pressure therapy in prevention and treatment of scar after deep burn in infants and young children.Methods A total of 86 children with deep burn who were treated in the wound treatment center of our hospital from December 2019 to December 2020 were selected as the research objects,and the children were divided into control group and experimental group according to the random number table method,with 43 cases in each group.The control group was given conventional anti-scar treatment,and the experimental group was given stepped pressure therapy on the basis of the control group.The scar severity,total incidence of adverse reactions,satisfaction of family members and compliance were compared between the two groups.Results After 3,6 and 12 months of treatment,the Vancouver Scar Scale(VSS)score of the experimental group was lower than that of the control group,and the difference was statistically significant(P<0.05).The total incidence of adverse reactions within 3 months after wound healing in the experimental group was lower than that in the control group,and the difference was statistically significant(P<0.05);there was no significant difference in the total incidence of adverse reactions between the two groups at 4-6 months and 7-12 months after wound healing(P>0.05).After 3,6 and 12 months of treatment,the satisfaction of family members of the experimental group was higher than that of the control group,and the difference was statistically significant(P<0.05).The compliance rates of follow-up treatment and timely referral in the experimental group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Stepped pressure therapy has a good prevention and treatment effect on scar after deep burn in infants and young children.It can reduce the severity of scar,reduce the incidence of adverse reactions,and improve the satisfaction of family members and compliance rate of children.It is worthy of clinica
