摘　　要：目的:分析左卡尼汀治疗维持性血液透析低血压患者的效果。方法:选取2020年7月—2022年7月乌鲁木齐市中医医院收治的90例维持性血液透析低血压患者作为研究对象。根据随机数表法将其分为对照组与研究组,各45例。对照组给予常规治疗,研究组在对照组基础上给予左卡尼汀治疗。比较两组治疗前后血压与心功能、微炎症指标、营养、贫血指标。结果:治疗后,研究组透析中最低舒张压、透析中最低收缩压、左心室射血分数均高于对照组,肌红蛋白、心肌肌钙蛋白I水平均低于对照组,差异有统计学意义(P<0.05)。治疗后,两组肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、超敏C反应蛋白(hs-CRP)水平均降低,研究组TNF-α、IL-6、hs-CRP水平均低于对照组,差异有统计学意义(P<0.05)。治疗后,两组白蛋白、血红蛋白、血细胞比容、总蛋白水平均高于对照组,研究组白蛋白、血红蛋白、血细胞比容、总蛋白水平均高于对照组,差异有统计学意义(P<0.05)。结论:左卡尼汀治疗维持性血液透析低血压能有效改善患者的血压与心功能,同时还能调节机体微炎症状态与营养、贫血指标,利于促进病情转归。Objective:To analyze the effect of Levocarnitine in the treatment in patients with maintenance hemodialysis hypotension.Method:A total of 90 patients with maintenance hemodialysis hypotension treated in Urumqi Traditional Chinese Medicine Hospital from July 2020 to July 2022 were selected as the study objects.According to random number table method,they were divided into control group and study group,45 cases in each group.The control group received conventional treatment,and the study group received Levocarnitine treatment on the basis of the control group.Blood pressure,heart function,microinflammation index,nutrition and anemia index were compared between the two groups before and after treatment.Result:After treatment,the minimum diastolic blood pressure during dialysis,minimum systolic blood pressure during dialysis and left ventricular ejection fraction of the study group were higher than those of the control group,and the levels of myoglobin and myocardial troponin I were lower than those of the control group,the differences were statistically significant(P<0.05).After treatment,the levels of tumor necrosis factor-α(TNF-α),interleukin-6(L-6)and hypersensitive C-reactive protein(hs-CRP)were decreased in both groups,and the levels of TNF-α,IL-6 and hs-CRP in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).After treatment,the levels of albumin,hemoglobin,hematocrit and total protein in the two groups were higher than those before treatment,and the levels of albumin,hemoglobin,hematocrit and total protein in study group were higher than those in control group,the differences were statistically significant(P<0.05).Conclusion:In the treatment of maintenance hemodialysis hypotension,Levocarnitine can effectively improve the blood pressure and cardiac function of patients,and also regulate the microinflammatory state of the body,nutrition and anemia indicators,which is conducive to promoting the outcome of the disease.
