B超引导下动静脉内瘘球囊扩张成形术在自体动静脉内瘘狭窄患者中的应用效果

摘　　要：目的:探究B超引导下动静脉内瘘(AVF)球囊扩张成形术在自体AVF狭窄患者中的应用效果。方法:选取2022年1—10月安顺市平坝人民医院收治的86例自体AVF狭窄患者。根据随机数表法将其分为对照组和观察组,各43例。对照组给予内瘘切除重建术治疗,观察组给予B超引导下AVF球囊扩张成形术。比较两组术后3个月、6个月内瘘通畅率,术前、术后3 d超声参数,并发症,术前、术后3个月生活质量及满意度。结果:观察组术后3个月、6个月内瘘通畅率高于对照组,差异有统计学意义(P<0.05)。术后3 d,观察组血管狭窄处内径、透析血流量、内瘘自然流量均高于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。术后3个月,观察组躯体功能、心理功能、社会功能、物质生活状态评分均高于对照组,差异有统计学意义(P<0.05)。观察组总满意度高于对照组,差异有统计学意义(P<0.05)。结论:B超引导下AVF球囊扩张成形术可有效提高自体AVF狭窄患者内瘘通畅率,降低并发症风险,改善生活质量,提高患者满意度。Objective:To investigate the application effect of B-ultrasound guided balloon dilation plasty operation of arteriovenous fistula(AVF)in patients with autologous arteriovenous fistula stenosis.Method:A total of 86 patients with autogenous AVF stenosis admitted to Pingba People's Hospital of Anshun City from January to October 2022 were selected.They were divided into control group and observation group according to random number table method,with 43 cases in each group.The control group was given internal fistula resection and reconstruction treatment,and the observation group was given B-ultrasound guided balloon dilation plasty operation of AVF.Internal fistula patency rate 3 months and 6 months after surgery,preoperative and postoperative 3 d ultrasound parameters,complications,preoperative and postoperative 3 months quality of life and satisfaction were compared between the two groups.Result:The internal fistula patency rates in the observation group were higher than those in the control group at 3 months and 6 months after operation,the differences were statistically significant(P<0.05).At 3 d after operation,the inner diameter of the blood vessel stenosis site,dialysis blood flow,and natural flow of the internal fistula in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).The incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Three months after operation,the scores of physical function,psychological function,social function,and material life status in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).The overall satisfaction of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:B-ultrasound guided balloon dilation plasty operation of AVF can effectively improve the patency rate of internal f
