Hem-o-lok夹闭与丝线结扎在腹腔镜阑尾切除术中的临床效果评价

摘　　要：目的:比较Hem-o-lok夹闭与丝线结扎在腹腔镜阑尾切除术中的临床效果。方法:选取2021年3月—2022年3月于沛县人民医院行腹腔镜阑尾切除术的96例患者作为研究对象,采用随机数表法将患者分为对照组与观察组,各48例。对照组采用丝线结扎,观察组采用Hem-o-lok夹闭。比较两组围手术期指标、术后疼痛程度和并发症发生情况。结果:观察组手术总时间短于对照组,术中出血量少于对照组,住院费用高于对照组,差异有统计学意义(P<0.05);两组术后首次下床活动时间、术后首次肛门排气时间、住院总时间比较,差异无统计学意义(P>0.05)。两组术后不同时间视觉模拟评分法(VAS)评分比较,差异无统计学意义(P>0.05)。观察组术后并发症发生率为8.33%,低于对照组的25.00%,差异有统计学意义(P<0.05)。结论:Hem-o-lok夹闭相较于丝线结扎应用于腹腔镜阑尾切除术中,具有缩短手术时间、减少术中出血量、降低术后并发症发生率等优点。但在临床中需综合考虑术者技术水平、患者经济条件等因素以选择合理的手术方法。Objective:To compare the clinical effect of Hem-o-lok clipping and silk ligation in laparoscopic appendectomy.Method:A total of 96 patients who underwent laparoscopic appendectomy in Pei County People's Hospital from March 2021 to March 2022 were selected as the study objects,and they were divided into the control group and the observation group by random number table method,48 cases in each group.The control group was given silk ligation,and the observation group was given Hem-o-lok clipping.The perioperative indicators,postoperative pain degree and complications were compared between the two groups.Result:The total operation time of the observation group was shorter than that of the control group,the intraoperative blood loss was less than that of the control group,and the hospitalization cost was higher than that of the control group,the differences were statistically significant(P<0.05);there were no significant differences in the first time of getting out of bed after operation,the first time of anal exhaust after operation and the total length of hospital stay between two groups(P>0.05).There were no significant difference in visual analogue scale(VAS)score between two groups at different time after operation(P>0.05).The incidence of postoperative complications in the observation group was 8.33%,which was lower than 25.00%in the control group,and the difference was statistically significant(P<0.05).Conclusion:Compared with silk ligation,Hem-o-lok clipping in laparoscopic apperdectomy has the advantages of shortening operation time,reducing intraoperative blood loss and the incidence of postoperative complications.However,in clinical practice,factors such as the technical level of the surgeon and the economic conditions of patients need to be considered comprehensively to choose a reasonable surgical method.
