顺铂腹腔灌注联合紫杉醇静脉滴注治疗晚期卵巢上皮癌的效果分析

摘　　要：目的:分析顺铂腹腔灌注联合紫杉醇静脉滴注治疗晚期卵巢上皮癌的效果。方法:回顾性分析2019年6月4日—2020年5月1日福建省妇幼保健院收治的84例晚期卵巢上皮癌患者的临床资料。根据顺铂给药方式的不同分为观察组和对照组,各42例。对照组先给予紫杉醇静脉滴注治疗,24 h后,给予顺铂静脉滴注治疗,观察组先给予紫杉醇静脉滴注治疗,24 h后,给予顺铂腹腔灌注治疗。比较两组治疗效果、随访1年死亡情况、治疗期间不良反应发生情况及治疗前后肿瘤标志物水平。结果:观察组缓解率为28.57%,显著高于对照组的9.52%,差异有统计学意义(P<0.05)。随访1年,观察组死亡2例(4.76%),少于对照组的8例(19.05%),两组比较差异有统计学意义(P<0.05)。观察组治疗期间不良反应发生率为14.29%,显著低于对照组的38.10%,差异有统计学意义(P<0.05)。两组治疗前肿瘤标志物水平比较,差异无统计学意义(P>0.05);观察组治疗后糖类抗原199、癌胚抗原及糖类抗原125水平低于对照组,差异有统计学意义(P<0.05)。结论:顺铂腹腔灌注联合紫杉醇静脉滴注治疗晚期卵巢上皮癌的效果优于顺铂联合紫杉醇静脉滴注治疗,可有效缩小肿瘤直径,降低死亡率、不良反应发生率及肿瘤标志物水平。Objective:To analyze the effect of Cisplatin intraperitoneal perfusion combined with Paclitaxel intravenous drip in the treatment of advanced epithelial ovarian cancer.Method:The clinical data of 84 patients with advanced epithelial ovarian cancer who admitted to Fujian Provincial Maternity and Children's Hospital from June 4,2019 to May 1,2020 were retrospectively analyzed,they were divided into the observation group and the control group according to the different ways of Cisplatin administration,with 42 cases in each group.The control group was given Paclitaxel intravenous drip first,and Cisplatin intravenous drip after 24 h,the observation group was given Paclitaxel intravenous drip and Cisplatin intraperitoneal perfusion after 24 h.The treatment effect,1-year follow-up death,adverse reactions during treatment and tumor marker levels before and after treatment were compared between two groups.Result:The remission rate of the observation group was 28.57%,which was significantly higher than 9.52%of the control group,and the difference was statistically significant(P<0.05).Following up for 1 year,2 cases(4.76%)died in the observation group,which was less than 8 cases(19.05%)in the control group,and the difference between two groups was statistically significant(P<0.05).The incidence of adverse reactions in the observation group during the treatment was 14.29%,which was significantly lower than 38.10%in the control group,and the difference was statistically significant(P<0.05).There were no significant differences in tumor markers levels between two groups before treatment(P>0.05):after treatment,the levels of carbohydrate antigen 199,carcinoembryonic antigen and carbohydrate antigen 125 in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:The effect of Cisplatin intraperitoneal perfusion combined with Paclitaxel intravenous drip in the treatment of advanced epithelial ovarian cancer is better than that of Cisplatin combined with
