摘  要:目的:探究超声心动图联合心肌酶谱检测对原发性高血压患者心脏结构和功能改变的评估价值。方法:选取东莞三局医院2021年3月—2022年5月收治的60例原发性高血压患者,根据患者是否发生心肌肥厚分为心肌肥厚组和非心肌肥厚组,各30例。另选取同期于本院接受健康体检的30例作为对照组。所有研究参与者均接受超声心动图联合心肌酶谱检测,观察三组的收缩压(SBP)、舒张压(DBP)、心脏指数(CI)、心输出量(CO)、左心室射血分数(LVEF)、左心室收缩末期容积(ESV)、左心室舒张末期容积(EDV)、舒张末期心室间隔厚度(IVSd)、血清肌酸激酶同工酶(CK-MB)、肌酸激酶(CK)水平及不同节段室壁的舒张期、收缩期心内膜相对力。结果:心肌肥厚组SBP、DBP显著高于对照组及非心肌肥厚组,而非心肌肥厚组SBP、DBP高于对照组,差异有统计学意义(P<0.05)。心肌肥厚组CI、CO低于对照组及非心肌肥厚组,非心肌肥厚组CI、CO低于对照组,差异有统计学意义(P<0.05)。三组LVEF、ESV、EDV比较,差异无统计学意义(P>0.05)。心肌肥厚组IVSd大于对照组及非心肌肥厚组,非心肌肥厚组IVSd大于对照组,差异有统计学意义(P<0.05)。心肌肥厚组血清CK-MB、CK水平高于对照组及非心肌肥厚组,非心肌肥厚组CK-MB、CK高于对照组,差异有统计学意义(P<0.05)。三组舒张期前壁及左侧壁心内膜相对力比较,差异无统计学意义(P>0.05);心肌肥厚组舒张期前间隔、后间隔、后壁、下壁心内膜相对力均小于对照组及非心肌肥厚组,非心肌肥厚组上述指标均小于对照组,差异有统计学意义(P<0.05)。三组收缩期前间隔、后间隔、后壁、下壁及左侧壁心内膜相对力比较,差异无统计学意义(P>0.05);心肌肥厚组收缩期前壁心内膜相对力大于对照组及非心肌肥厚组,非心肌肥厚组大于对照组,差异有统计学意义(P<0.05)。结论:超声心动图联合心肌酶谱Objective:To explore the evaluation value of echocardiography combined with myocardial enzyme spectrum detection in cardiac structural and functional changes in patients with essential hypertension.Method:A total of 60 patients with essential hypertension admitted to Dongguan Sanju Hospital from March 2021 to May 2022 were selected.According to whether the patients had myocardial hypertrophy,they were divided into myocardial hypertrophy group and non-myocardial hypertrophy group,with 30 cases in each group.Another 30 patients who received physical examination in our hospital during the same period were selected as the control group.All study participants underwent echocardiography combined with myocardial enzyme spectrum detection.Systolic blood pressure(SBP),diastolic blood pressure(DBP),cardiac index(CI),cardiac output(CO),left ventricular ejection fraction(LVEF),left ventricular end-systolic volume(ESV),left ventricular end-diastolic volume(EDV),interventricular septal thickness at diastole(IVSd),serum creatine kinase isoenzyme(CK-MB),creatine kinase(CK)levels,and relative force of endocardium between different segments of ventricular wall during diastole and systole.Result:The SBP and DBP in the myocardial hypertrophy group were significantly higher than those in the control group and non-myocardial hypertrophy group,while the SBP and DBP in the non-myocardial hypertrophy group were higher than those in the control group,the differences were statistically significant(P<0.05).The CI and CO levels in the myocardial hypertrophy group were lower than those in the control group and non-myocardial hypertrophy group,while the CI and CO levels in the non-myocardial hypertrophy group were lower than those in the control group,the differences were statistically significant(P<0.05).There was no statistically significant difference in LVEF,ESV,and EDV among the three groups(P>0.05).The IVSd of the myocardial hypertrophy group was greater than that of the control group and non-myocardial hypertrophy group,while the IVSd o
