摘　　要：目的采用meta分析方法,研究阻塞性睡眠呼吸暂停低通气综合征(OSAHS)与主动脉夹层的关系,探究OSAHS是否为引起主动脉夹层发病的危险因素。方法通过检索中文数据库系统万方数据库和中国知网,英文数据库系统PubMed、Embase和Cochrane Library,获得关于OSAHS与主动脉夹层相关性的研究结论,并对其开展方法学质量评价和筛选。采用meta分析方法对所获得的数据进行分析。结果本次分析共涉及14篇研究,其中,病例对照类的研究5篇,横断面类研究9篇,共1526例患者。meta分析结果表明,在主动脉夹层患者中OSAHS病例共有507例,发病率为42.7%。病例对照研究的meta分析结论提示,与对照组相比,主动脉夹层组患者的OSAHS发病率(OR=2.63,95%CI:1.65~4.19,P<0.05)、中重度OSAHS发病率(OR=4.10,95%CI:2.75~6.11,P<0.05)、睡眠呼吸暂停低通气指数(均数差10.61,95%CI:7.36~13.86,P<0.05)均较高。结论OSAHS可能是主动脉夹层并发的危险因素。Objective To explore whether obstructive sleep apnea hypopnea syndrome(OSAHS)is a risk factor for aortic dissection based on meta analysis.Methods The Chinese database Wanfang and CNKI,and English databases PubMed,Embase and Cochrane Library were comprehensively searched to obtain the research results on the relationship between OSAHS and aortic dissection,and the methodological quality was evaluated and screened.The extracted data were analyzed by meta-analysis method.Results This analysis involved a total of 14 studies,including 5 case-control studies and 9 cross-sectional studies,with a total of 1526 patients.Meta-analysis showed that 507 patients with OSAHS were involved in aortic dissection,and the incidence rate was 42.7%.Meta-analysis of case-control study showed that,compared with the control group,the incidence rate of OSAHS(OR=2.63,95%CI:1.65-4.19,P<0.05),the incidence rate of moderate and severe OSAHS(OR=4.10,95%CI:2.75-6.11,P<0.05),sleep apnea hypopnea index(mean difference 10.61,95%CI:7.36-13.86,P<0.05)in the aortic dissection group were all higher.Conclusion OSAHS may be a risk factor for aortic dissection.
