经脐单孔腹腔镜与多孔腹腔镜在异位妊娠手术中的疗效分析

摘　　要：目的:探讨经脐单孔腹腔镜与多孔腹腔镜在异位妊娠手术中的应用效果。方法:选取2021年1月—2023年1月东莞市横沥医院收治的异位妊娠患者62例为观察对象,均行腹腔镜手术治疗,其中行多孔腹腔镜手术31例为多孔组,行经脐单孔腹腔镜手术31例为单孔组。比较两组患者手术指标、术后疼痛程度、切口满意度、手术并发症发生情况。结果:单孔组手术时间长于多孔组,出血量多于多孔组,住院时间、排气时间短于多孔组,差异有统计学意义(P<0.001);术后2、6、12、24h,单孔组疼痛评分均低于多孔组,差异有统计学意义(P<0.001);单孔组切口满意度评分高于多孔组,差异有统计学意义(P<0.001);两组手术并发症发生率比较,差异无统计学意义(P>0.05)。结论:经脐单孔腹腔镜手术治疗异位妊娠,可促进患者康复,减少创伤导致的疼痛,切口美观度高于多孔腹腔镜手术,手术并发症发生率低。Objective:To explore the application effect of transumbilical single-hole laparoscopy and porous laparoscopy in ectopic pregnancy surgery.Methods:A total of sixty-two patients with ectopic pregnancy treated in Donggua Hengli Hospital from January 2021 to January 2023 were selected as the observation objects,all of whom underwent laparoscopic surgery.Among them,thirty-one cases underwent porous laparoscopic surgery as the porous group,and thirty-one cases underwent transumbilical single hole laparoscopic surgery as the single hole group.The surgical indexes,postoperative pain,incision satisfaction and surgical complications were compared between the two groups.Results:The operation time of the single hole group was longer than that of the porous group,the blood loss was longer than that of the porous group,and the hospital stay and exhaust time were shorter than that of the porous group,the difference was statistically significant(P<0.001);At 2,6,12 and 24 h after surgery,pain scores in the single hole group were lower than those in the porous group,with statistical significance(P<0.001);The incision satisfaction score of the single hole group was higher than that of the porous group,and the difference was statistically significant(P<0.001);There was no significant difference in the incidence of surgical complications between the two groups(P>0.05).Conclusion:Transumbilical single-hole laparoscopic surgery for ectopic pregnancy can promote the recovery of patients,reduce the pain caused by trauma,the incision beauty is higher than that of porous laparoscopic surgery,and the incidence of surgical complications is lower.
