摘　　要：目的:分析血栓弹力图在急性早幼粒细胞白血病患者凝血功能评估中的应用价值。方法:选取2020年3月—2023年2月江门市中心医院血液内科收治的急性早幼粒细胞白血病患者85例作为观察组,按照有无出血分为出血组(38例)和无出血组(47例),选取同期健康体检者85例为对照组。比较观察组和对照组血栓弹力图相关参数[凝血反应时间(R值)、凝血形成时间(K值)、凝固角(Angle角)和血栓最大强度(MA值)]差异;比较出血组与无出血组R值、K值、Angle角、MA值差异。结果:观察组R值短于对照组,K值长于对照组,Angle角和MA值低于对照组,差异有统计学意义(P<0.05);出血组R值短于无出血组,K值长于无出血组,Angle角和MA值低于无出血组,差异有统计学意义(P<0.05)。结论:急性早幼粒细胞白血病患者凝血功能处于低凝状况,应用血栓弹力图观察凝血状况,对于治疗的评估有重要意义。Objective:To analyze the application value of thromboelastography in the assessment of coagulation function in patients with acute promyelocytic leukemia.Methods:85 patients with acute promyelocytic leukemia admitted to the Department of Hematology,Jiangmen Central Hospital from March 2020 to February 2023 were selected as the observation group.According to the presence or absence of bleeding,they were divided into bleeding group(38 cases)and no bleeding group(47 cases).85 healthy persons who underwent physical examination during the same period were selected as the control group.Compare the differences in thromboelastography-related parameters[coagulation reaction time(R value),coagulation formation time(K value),coagulation angle(Angle angle),and maximum thrombus intensity(MA value)]between the observation group and the control group.Compare the differences in R value,K value,Angle angle and MA value between the bleeding group and the non-bleeding group.Results:The R value of the observation group was shorter than that of the control group,the K value was longer than that of the control group,and the Angle angle and MA value were lower than those of the control group;The differences were statistically significant(P<0.05);The R value of the bleeding group was shorter than that of the non-bleeding group,the K value was longer than that of the non-bleeding group,and the Angle angle and MA value were lower than those of the non-bleeding group;The differences were statistically significant(P<0.05).Conclusion:The coagulation function of patients with acute promyelocytic leukemia is in a hypocoagulable state.The use of thromboelastography to observe the coagulation status is of great significance for the evaluation of treatment.
