清热养阴汤治疗心肝火旺型桥本氏甲亢临床观察

摘　　要：目的探讨清热养阴汤治疗心肝火旺型桥本氏甲亢的临床疗效。方法选取符合纳入标准的98例桥本氏甲亢患者以随机数字表法分为对照组、观察组各49例,对照组予常规西药治疗,观察组在此基础上予清热养阴汤口服治疗,比较2组疗效、中医证候积分、甲状腺激素及抗体水平。结果观察组总有效率高于对照组(P<0.05)。治疗3个月后,观察组中医证候积分、游离三碘甲状腺原氨酸(FT3)和游离四碘甲状腺原氨酸(FT4)水平、甲状腺过氧化物酶抗体(TPOAb)水平低于对照组(P<0.05);观察组促甲状腺激素(TSH)水平高于对照组(P<0.05)。结论清热养阴汤治疗心肝火旺型桥本氏甲亢临床疗效显著,能充分缓解临床症状,改善甲状腺激素水平,值得临床应用。Objective To explore the clinical effect of Qingre Yangyin decoction in the treatment of Hashimoto’s hyperthyroidism with exuberant fire in heart and liver type.Methods A total of 98 patients with Hashimoto’s hyperthyroidism who met the inclusion criteria were selected and randomly divided into the control group and the observation group,with 49 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with Qingre Yangyin decoction orally on the basis of the control group.The curative effect,TCM syndrome score,thyroid hormone and antibody levels were compared between two groups.Results The total effective rate of the observation group was higher than the control group(P<0.05).After 3 months of treatment,the TCM syndrome scores,free triiodothyronine(FT3)level and free tetraiodothyronine(FT4)level,and thyroid peroxidase antibody(TPOAb)level in the observation group were lower than those in the control group(P<0.05).The level of thyroid stimulating hormone(TSH)in the observation group was higher than that in the control group(P<0.05).Conclusions Qingre Yangyin decoction has significant clinical curative effect in the treatment of Hashimoto’s hyperthyroidism with the type of exuberant fire in heart and liver,it can fully relieve clinical symptoms and improve the thyroid hormone level,which is worthy of clinical application.
