摘　　要：目的 观察针刺联合康复运动治疗对脑卒中后认知障碍、运动功能和应激反应的影响。方法 选取2019年4月至2022年4月诊治的90例脑卒中患者作为研究对象。根据随机数字表法将患者分为对照组45例和联合组45例,对照组采用康复运动治疗,联合组采用针刺联合康复运动治疗。比较两组神经功能[美国国立卫生研究院卒中量表(NIHSS)评分]、认知功能[蒙特利认知评估量表(MoCA)评分]、运动功能[Fugl-Meyer运动功能评定量表评分(FMA评分)]、氧化应激指标[丙二醛(MDA)、谷胱甘肽过氧化物酶(GSH-Px)、8-羟基脱氧鸟苷酸(8-OHdG)]、疗效以及不良反应发生率。结果 联合组总有效率为95.56%,高于对照组的73.33%(P<0.05)。治疗后联合组NIHSS评分、MDA、8-OHdG水平均低于对照组(P<0.05),MoCA评分、FMA评分、GSH-Px水平均高于对照组(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论 针刺联合康复运动治疗脑卒中患者可改善神经功能,提高认知能力和运动功能,降低氧化应激损伤,疗效显著,安全性高。Objective To observe the effects of acupuncture combined with rehabilitation exercise on cognitive impairment,motor function and stress response after stroke.Methods From April 2019 to April 2022,90 stroke patients treated at our hospital were selected.Using a random number table,the patients were divided into a control group of 45 cases and a combined group of 45 cases.The control group received rehabilitation exercises,while the combined group received acupuncture combined with rehabilitation exercises.The two groups were compared in terms of neurological function using National Institutes of Health Stroke Scale(NIHSS)score,cognitive function using Montreal Cognitive Assessment(MoCA)score,motor function using Fugl-Meyer Motor Assessment Scale(FMA)score,oxidative stress markers using malondialdehyde(MDA),glutathione peroxidase(GSH-Px),and 8-hydroxy-2′-deoxyguanosine(8-OHdG),therapeutic effects,and the incidence of adverse reactions.Results The total effective rate in the combined group was 95.56%,which was higher than the control group′s rate of 73.33%(P<0.05).After treatment,the NIHSS score,MDA,and 8-OHdG levels in the combined group were lower than those in the control group(P<0.05),while the MoCA score,FMA score,and GSH-Px level were higher than those in the control group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Acupuncture combined with rehabilitation exercise for stroke patients can improve neurological function,enhance cognitive and motor abilities,reduce oxidative stress damage,and achieve significant therapeutic effects with a high level of safety.
