自拟中药溻渍方联合红外线理疗灯治疗风寒湿痹证类风湿关节炎的临床观察及机理探讨

摘　　要：目的:研究自拟中药溻渍方联合红外线理疗灯治疗风寒湿痹证类风湿关节炎(RA)的机理及临床疗效。方法:选取2020年1月~2021年12月于张家口市中医院就诊的风寒湿痹证RA患者86例,随机数字法分为对照组和治疗组,每组43例;对照组采用口服药物治疗,治疗组在对照组基础上采用自拟中药溻渍方联合红外线理疗灯治疗,比较两组中医证候积分、临床疗效、疾病活动程度(DAS28)评分、健康评价问卷评分(HAQ)、C反应蛋白(CRP)、血沉(ESR)及类风湿因子(RF)的水平。结果:治疗组的总有效率(93.02%)显著高于对照组的总有效率(69.77%)(P<0.05);两组患者治疗后各项中医证候积分及总分明显低于治疗前,其中治疗组治疗后上述评分均低于对照组(P<0.05);两组治疗后DAS28评分及HAQ评分显著低于对照组,其中治疗组治疗后上述评分显著低于对照组(P<0.05);两组治疗后CRP、ESR及RF水平较治疗前显著下降,其中治疗组治疗后CRP、ESR及RF水平显著低于对照组(P<0.05);两组患者治疗期间未见明显不良反应,均顺利完成治疗。结论:自拟中药溻渍方联合红外线理疗灯治疗风寒湿痹证RA患者可有效缓解患者临床症状,减低患者类风湿因子、C-反应蛋白、血沉等活动性指标,疗效显著。Objective:To study the mechanism and clinical effect of external liquid application of external liquid application of self-made traditional Chinese medicine combined with infrared physiotherapy lamp in the treatment of rheumatoid arthritis(RA)with wind-cold-dampness arthralgia syndrome.Methods:A total of 86patients with RA and wind-cold-dampness arthralgia syndrome who were treated in Zhangjiakou Hospital of Traditional Chinese Medicine from January 2020to December 2021were selected,randomly divided into control group and atreatment group by random number table method with 43cases in each group.Patients in the control group were treated with oral medication.On this basis,patients in the treatment group were treated with external liquid application ofself-made traditional Chinese medicine prescription combined with infrared physiotherapy lamp therapy.TCM syndrome scores,clinical efficacy,disease activity score(DAS28),health assessment questionnaire score(HAQ),and the levels of C-reactive protein(CRP),erythrocyte sedimentation rate(ESR)and rheumatoid factor(RF)were compared between the two groups.Results:The total effective rate in the treatment group(93.02%)was significantly higher than that in the control group(69.77%)(P<0.05).After treatment,the scores of TCM syndrome and the total scores in the two groups were significantly lower than those before treatment,and the scores in the treatment group were lower than those in the control group(P<0.05),the DAS28scores and HAQ scores of the two groups decreased,and the scores in the treatment group were significantly lower than those in the control group after treatment(P<0.05).After treatment,the levels of CRP,ESR and RF in the two groups decreased,and the levels in the treatment group were significantly lower than those in the control group(P<0.05).No significant adverse reactions were observed during the treatment period in both groups,and all patients successfully completed the treatment.Conclusion:External liquid application of self-made traditional Chinese medicin
