中药外敷、穴位按摩联合局部封闭治疗哺乳期急性乳腺炎对血清IgG、IgM、IgA表达的影响

摘　　要：目的:探讨中药外敷、穴位按摩联合局部封闭治疗哺乳期急性乳腺炎对血清免疫球蛋白G(IgG)、免疫球蛋白M(IgM)、免疫球蛋白A(IgA)表达的影响。方法:依据随机数字表法,将2018年9月~2020年9月本院收治的126例哺乳期急性乳腺炎患者分成2组,每组63例,对照组行青霉素静脉滴注,研究组行中药外敷+穴位按摩+局部封闭治疗。评价两组疗效,统计相关症状消失时间及通乳时间,检测IgG、IgM、IgA、白细胞介素(IL-8)、肿瘤坏死因子-α(TNF-α)水平。结果:研究组总有效率95.24%,高于对照组71.43%(χ^(2)=12.857,P=0.000)。研究组皮肤发红、肿块、疼痛、体温等症状消失时间及通乳时间均短于对照组(P<0.05)。治疗后,两组血清IgG、IgM、IgA表达水平均下降(P<0.05),且研究组血清IgG、IgM、IgA表达水平均低于对照组(t=16.648、5.831、9.184,P均<0.05)。治疗后,两组IL-8、TNF-α水平均下降(P<0.05),且研究组IL-8、TNF-α水平均低于对照组(t=22.374、7.261,P均<0.05)。结论:中药外敷+穴位按摩+局部封闭治疗哺乳期急性乳腺炎疗效显著,能减轻症状、促进排乳,下调免疫因子、炎性因子表达。Objective:To investigate the effect of external application of traditional Chinese medicine+acupoint massage+local block therapy on the expression of serum immunoglobulin G(IgG),immunoglobulin M(IgM),and immunoglobulin A(IgA)in patients with lactation acute mastitis.Methods:According to the random number table method,126cases of lactation acute mastitis patients in our hospital from September 2018to September 2020were divided into two groups,with 63cases in each group.The control group was given penicillin intravenous drip,and the study group was treated with external application of traditional Chinese medicine+acupoint massag+local blocking therapy.The curative effect of the two groups was evaluated,the disappearance time of related symptoms and the mammary gland dredge time were counted,and the levels of IgG,IgM,IgA,nterleukin(IL-8)and tumor necrosis factor-α(TNF-α)were detected.Results:The total effective rate of the study group was 95.24%,which was higher than 71.43%of the control group(χ^(2)=12.857,P=0.000).The disappearance time of skin redness,mass,pain,body temperature and other symptoms,and mammary gland dredge time in the study group were shorter than those in the control group(P<0.05).After treatment,the expression levels of serum IgG,IgM and IgA in the two groups decreased(P<0.05),and the expression levels of serum IgG,IgM and IgA in the study group were lower than those in the control group(t=16.648,5.831,9.184,all P<0.05).After treatment,the levels of IL-8and TNF-αin the two groups decreased(P<0.05),and the levels of IL-8and TNF-αin the study group were lower than those in the control group(t=22.374,7.261,all P<0.05).Conclusion:External application of traditional Chinese medicine+acupoint massage+local block therapy has significant effect on acute mastitis during lactation,which can relieve symptoms,promote milk discharge,and down-regulate the expression of immune factors and inflammatory factors.
