凉血消疕汤加减联合窄谱中波紫外线治疗寻常血热型银屑病临床疗效观察

摘　　要：目的:探讨凉血消疕汤加减联合窄谱中波紫外线(NB-UVB)治疗寻常血热型银屑病的临床疗效。方法:选取2018年1月~2020年1月陕西省中医医院收治的168例PV患者,根据乱数表法分为两组,对照组(n=84)接受NB-UVB治疗,观察组(n=84)接受凉血消疕汤加减联合NB-UVB治疗,对比两组患者临床疗效、治疗后中医证候积分、PASI评分和外周血辅助性T细胞17(Th17)、调节性T细胞(Treg)、Th17/Treg及血清肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、IL-17水平、不良反应。结果:观察组临床疗效明显高于对照组(P<0.05);治疗后观察组中医证候积分、PASI评分和外周血Th17、Th17/Treg及血清TNF-α、IL-6、IL-17水平明显降低,Treg细胞百分比明显提升(P<0.05);两组患者不良反应发生率对比无差异(P>0.05)。结论:凉血消疕汤加减联合NB-UVB治疗血热型PV疗效确切,可明显改善中医证候积分,缩小皮损面积,降低皮损严重程度,其机制可能与调节Th17/Treg平衡和抑制炎症反应有关。Objective:To explore the clinical efficacy of Liangxue Xiaobian Decoction combined with narrow-band ultraviolet(NB-UVB)in the treatment of vulgar blood-heat psoriasis.Methods:168PV patients admitted to our hospital(January 2018to January 2020)were selected and divided into two groups according to the random number table method,the control group(n=84)received NB-UVB treatment,and the observation group(n=84)Received Liangxue Xiaobian Decoction combined with NB-UVB treatment,Clinical efficacy,TCM syndrome score,PASI score,peripheral blood helper T cells 17(Th17),regulatory T cells(Treg),Th17/Treg,and serum levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),IL-17and adverse reactions were compared between the two groups.Results:The clinical efficacy of the observation group was significantly higher than that of the control group(P<0.05);After treatment,the TCM syndrome score,PASI score,peripheral blood Th17,Th17/Treg and serum TNF-α,IL-6,IL-17levels of the observation group decreased,and the percentage of Treg cells increased significantly(P<0.05);There was no difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The combination of Liangxue Xiaoyi Decoction combined with NB-UVB has a definite effect in the treatment of blood-heat PV,it can significantly improve the TCM syndrome score,reduce the area of skin lesions,and reduce the severity of skin lesions.The mechanism may be related to regulating Th17/Treg balance and inhibiting inflammation the reaction is related.
