清肺化痰汤联合西药对慢性肺源性心脏病(痰瘀阻肺证)患者心功能及动脉血气的影响

摘　　要：目的:观察清肺化痰汤联合西药对慢性肺源性心脏病(CPHD,痰瘀阻肺证)患者心功能及动脉血气的影响。方法:选取2020年6月~2021年6月本院收治的94例CPHD患者(痰瘀阻肺证)作为观察对象,随机数字表及随机余数法分为联合组(n=47,清肺化痰汤+西药)与对照组(n=47,西药)。比较两组临床疗效,比较治疗前、治疗14d后两组中医证候积分、心功能、动脉血气分析、血液流变学、血清相关因子水平。结果:治疗14d后,联合组临床总有效率为95.74%,高于对照组的82.98%(P<0.05)。治疗14d后,两组中医证候(咳嗽、咳痰、气喘、呼吸困难、水肿、唇甲紫绀)积分、血液流变学指标(Fbg、WBHSV、WBLSV、PV)、血清相关因子(CPP、NT-proBNP)、部分动脉血气分析指标(PaCO_(2))均低于治疗前,且联合组低于对照组(P<0.05);两组心功能指标(LVEF、CI、E/A)、PaO2水平均高于治疗前,且联合组高于对照组(P<0.05)。结论:清肺化痰汤+西药在CPHD患者(痰瘀阻肺证)中应用效果显著,可降低中医证候积分,改善心功能及动脉血气分析,降低血液粘稠度,改善预后。Objective:To observe the effects of Qingfei Huatan decoction combined with western medicine on cardiac function and arterial blood gas in patients with chronic pulmonary heart disease(CPHD)of phlegm and blood stasis syndrome.Methods:94patients with CPHD(phlegm and blood stasis syndrome)treated in the hospital from June 2020to June 2021were selected as the observation subjects,and they were divided into combined group(n=47,Qingfei Huatan decoction+western medicine)and control group(n=47,western medicine)by the random number table method and random remainder method.The clinical efficacy,and scores of TCM syndromes,cardiac function,arterial blood gas analysis,hemorheology and serum related factors before treatment and after 14days of treatment were compared between the two groups.Results:After 14days of treatment,the total clinical effective rate in combined group was 95.74%,which was higher than 82.98%in control group(P<0.05).After 14days of treatment,the scores of TCM syndromes(cough,expectoration,asthma,dyspnea,edema,cyanosis),hemorheological indicators(Fbg,WBHSV,WBLSV,PV),serum-related factors(CPP,NT-proBNP)and some arterial blood gas analysis indicator(PaCO_(2))of the two groups were lower than those before treatment,and lower in the combined group than in the control group(P<0.05).The cardiac function indicators(LVEF,CI,E/A)and PaO,of both groups were higher than those before treatment,and the indicators were higher in combined group than control group(P<0.05).Conclusion Qingfei Huatan decoction+western medicine has a significant application effect on patients with CPHD(phlegm and blood stasis syndrome),and it can reduce TCM syndromes scores,improve cardiac function and arterial blood gas analysis,reduce blood viscosity,and improve prognosis.
