单硝酸异山梨酯注射液联合双水平无创正压通气治疗急性左心衰伴Ⅱ型呼吸衰竭患者的临床效果

摘　　要：目的 探讨单硝酸异山梨酯注射液联合双水平无创正压通气(BiPAP)治疗急性左心衰伴Ⅱ型呼吸衰竭患者的效果。方法 90例急性左心衰伴Ⅱ型呼吸衰竭患者随机分为两组,对照组采用单硝酸异山梨酯注射液治疗,观察组在对照组基础上采用BiPAP治疗,比较两组的治疗效果、心功能及呼吸功能。结果 治疗后,观察组的总有效率以及SV、 LVEF、 CI、 PaO_(2)、 SaO_(2)水平均高于对照组,PaCO_(2)水平低于对照组(P<0.05)。结论 单硝酸异山梨酯注射液联合BiPAP治疗急性左心衰伴Ⅱ型呼吸衰竭可明显改善患者的心功能及呼吸功能。Objective To explore the effect of isosorbide mononitrate injection combined with bi-level non-invasive positive airway pressure ventilation(BiPAP)in the treatment of patients with acute left heart failure complicated with typeⅡrespiratory failure.Methods 90 patients with acute left heart failure complicated with typeⅡrespiratory failure were randomly divided into two groups.The control group was treated with isosorbide mononitrate injection,while the observation group was treated with BiPAP on the basis of the control group.The treatment effect,cardiac function and respiratory function were compared between the two groups.Results After treatment,the total effective rate and the levels of SV,LVEF,CI,PaO_(2) and SaO_(2) in the observation group were higher than those in the control group,and the PaCO_(2) level was lower than that in the control group(P<0.05).Conclusions Isosorbide mononitrate injection combined with BiPAP in the treatment of acute left heart failure complicated with typeⅡrespiratory failure can obviously improve patients'cardiac function and respiratory function.
