重度急性有机磷农药中毒患者血清超敏肌钙蛋白Ⅰ、胱抑素-C水平变化及其对预后的评估价值

摘　　要：目的探讨重度急性有机磷农药中毒(SAOPP)患者血清超敏肌钙蛋白Ⅰ(hs-cTnI)、胱抑素-C(Cys-C)水平变化及其对预后的评估价值。方法选取72例SAOPP患者作为观察组,60例健康者作为对照组。比较两组的血清hs-cTnI、Cys-C水平,比较不同预后患者的血清hs-cTnI和Cys-C水平,分析hs-cTnI、Cys-C与二者联合对预后的评估价值。结果观察组的血清hs-cTnI、Cys-C水平均高于对照组,预后不良患者的hs-cTnI、Cys-C水平均高于预后良好患者(P<0.05)。ROC曲线显示,hs-cTnI、Cys-C联合评估SAOPP患者预后的AUC为0.934,高于各指标单独评估(P<0.05)。结论SAOPP患者的血清hs-cTnI、Cys-C水平呈异常高表达,hs-cTnI、Cys-C均可用于预后的评估,联合检测的评估价值更高。Objective To explore the changes of serum high-sensitivity troponinⅠ(hs-cTnI)and Cystatin-C(Cys-C)levels in patients with severe acute organophosphorus pesticide poisoning(SAOPP)and their evaluation value for prognosis.Methods 72 patients with SAOPP were selected as the observation group,and 60 healthy people were selected as the control group.The levels of serum hs-cTnI and Cys-C were compared between the two groups,and between patients with different prognosis.The evaluation value of hs-cTnI,Cys-C and their combination for prognosis were analyzed.Results The levels of serum hs-cTnI and Cys-C in the observation group were higher than those in the control group,and the levels of hs-cTnI and Cys-C in patients with poor prognosis were higher than those in patients with good prognosis(P<0.05).ROC curve showed that the AUC of hs-cTnI combined with Cys-C in evaluating the prognosis of SAOPP patients was 0.934,higher than that of each index alone(P<0.05).Conclusions The expression levels of serum hs-cTnI and Cys-C in SAOPP patients are abnormally high.Both hs-cTnI and Cys-C can be used for prognosis evaluation,and the combined detection has higher evaluation value.
