摘　　要：抗中性粒细胞胞质抗体相关性血管炎(anti-neutrophil cytoplasm antibody-associated vasculitis,AAV)是一类全身性自身免疫性疾病,可导致肾脏、肺等多种重要器官受损,其治愈难度大,现有的治疗手段并未能给患者带来长期良好的治疗效果,严重影响了患者的生活质量。近年来AAV的发病率和流行率呈现上升趋势,如何防治AAV至今仍是一个值得研究探讨的问题。AAV的致病因素尚未明确,且影响疾病复发的因素较多。相关证据表明,多基因遗传易感性、表观遗传影响和环境危险因素在AAV的发病中存在复杂的相互作用。现有的国外相关研究显示,遗传易感性的患者在环境因素的影响下更容易发生AAV,且环境因素对AAV的复发存在一定影响,所以环境危险因素对AAV的影响在近些年来也逐渐被重视。我们将对环境相关危险因素对AAV发病影响的研究进展做以下综述。As a common systemic autoimmune disease,anti-neutrophil cytoplasm antibody(ANCA)-associated vasculitis(AAV)may cause an impairment of kidney,lung and other important organs.Clinical cure is rather difficult.The previous therapeutic strategies have failed to provide long-term efficacy for patients so that there are lower standards of living.The incidence and prevalence of AAV has spiked in recent years.So how to prevent and manage AAV is still a hot topic.The exact etiology of AAV remains elusive and its recurrence is influenced by many factors.Growing studies have demonstrated that polygenic predisposition,epigenetic effects and environmental risk factors play some complex roles in the pathogenesis of AAV.As suggested by previous related foreign researches,patients with genetic susceptibility are more prone to AAV under the impact of environmental factors and environmental factors have some effects on the recurrence of AAV.This review summarized the latest researches of the impact of environment-related risk factors on the incidence of ANCA-associated vasculitis.
