消风散加减联合康复新液、蒙脱石散治疗婴幼儿尿布皮炎40例临床观察

摘　　要：目的 观察消风散加减外洗联合康复新液、蒙脱石散外涂治疗婴幼儿尿布皮炎的临床疗效。方法 选取2020年6月至2021年6月怀化市妇幼保健院儿科和儿科重症监护室收治的尿布皮炎患儿80例,按照入院顺序的单双号分为对照组和治疗组,各40例。对照组温水清洗皮肤后,用纸巾擦干,予炉甘石洗剂涂擦患处,待干燥后再涂擦适量湿润烧伤膏。治疗组予消风散加减外洗(年龄大且配合的患儿可坐浴,不配合外洗的患儿直接用棉签蘸取适量药液涂抹臀部);直接用纸巾擦干后0.5 h,予康复新液喷洒,喷洒面积要尽量覆盖到皮疹、溃疡面;待康复新液干燥后0.5 h,外涂蒙脱石散,蒙脱石散需用温水调成稀糊状,每包兑10 mL左右温水。2组均每日治疗4~5次。结果 对照组总有效率为77.5%(31/40),治疗组为97.5%(39/40),2组比较,差异有统计学意义(P<0.05)。治疗组皮疹消退时间、渗液消失时间、创面愈合时间明显短于对照组,经统计学分析,差异有统计学意义(P<0.05)。2组在治疗期间均未发生继发性皮肤过敏、皮疹加重等不良反应。结论 中药消风散加减外洗联合康复新液、蒙脱石散外涂治疗婴幼儿尿布皮炎有很好的临床疗效,不仅能促进皮疹消退、溃疡创面愈合,而且无不良反应,值得临床推广应用。Objective To observe the clinical efficacy of modified Xiaofeng San(消风散) combined with Kangfuxin liquid and montmorillonite powder in the treatment of infant diaper dermatitis.Methods From June 2020 to June 2021,80 children with diaper dermatitis were selected from the pediatrics department and pediatric intensive care units of Huaihua Maternal and Child Health Hospital.They were divided into a control group and a treatment group according to single and double numbers in order of admission,with 40 cases in each group.After washing the skin with warm water in the control group,wipe it dry with a tissue and apply calamine lotion to the affected area.After drying,apply an appropriate amount of moist burn ointment.The treatment group was treated with modified Xiaofeng San(消风散) external washing(elderly children who cooperate can take sitz baths,while children who do not cooperate can directly use a cotton swab to dip in an appropriate amount of medication and apply it to their buttocks).After directly wiping with a tissue for 0.5 hours,spray the Kangfuxin liquid,covering the area of the rash and ulcer as much as possible.After the Kangfuxin liquid was dried for 0.5 hours,apply montmorillonite powder externally.The montmorillonite powder needs to be mixed into a thin paste with warm water,and mix about 10 mL of warm water to each package.Both groups were treated 4-5 times a day.Results The total effective rate of the control group was 77.5%(31/40),while that of the treatment group was 97.5%(39/40).The difference between the two groups was statistically significant(P<0.05).The treatment group had significantly shorter time for rash resolution,exudate disappearance,and wound healing compared to the control group,and the difference was statistically significant(P<0.05).Both groups did not experience secondary skin allergies or worsening rash during the treatment period.Conclusion Traditional Chinese medicine Xiaofeng San combined with Kangfuxin liquid and montmorillonite powder has good clinical efficacy in th
