摘 要:糖尿病肾病(diabetes kidney disease,DKD)是糖尿病的主要并发症之一,是慢性肾脏病和终末期肾病的主要原因,其患病率在全球范围内不断上升。细胞焦亡(pyroptosis)是一种程序性细胞死亡,其特征是细胞肿胀、细胞内容物泄漏和释放炎症因子。细胞焦亡与糖尿病肾病密切相关,而其调控机制尚不明确。非编码RNA(non-coding RNA,ncRNA),如微小RNA(microRNA,miRNA)、长链非编码RNA(long non-coding RNA,lncRNA)和环状RNA(circRNA)在细胞焦亡的生物学调控中起重要作用。研究发现,ncRNA可直接或间接调控细胞焦亡通路,也可形成竞争性内源RNA(competing endogenous RNAs,ceRNA)网络调控细胞焦亡,进而影响DKD的发展。该文总结了ncRNA在DKD细胞焦亡中的调控作用以及中医药通过调控细胞焦亡相关通路延缓DKD的研究进展,可为DKD的发病机制和临床治疗提供研究方向和思路。Diabetic kidney disease(DKD)is one of the major complications of diabetes.It was also recognized as the leading cause of chronic kidney disease and end-stage renal disease.Its prevalence has been increasing worldwide.Pyroptosis is a form of programmed cell death,which is characterized by cell swelling,leakage of cell contents and the release of inflammatory factors.Pyroptosis has been shown to be closely associated with diabetic kidney disease,but its regulatory mechanisms are still unclear.Non-coding RNAs(ncRNAs)such as microRNAs,long non-coding RNAs and circular RNAs play an important role in the biological regulation of pyroptosis.It has been found that ncRNAs can directly or indirectly regulate the pyroptosis pathway and form a competitive endogenous RNAs(ceRNA)networks to regulate pyroptosis,which in turn affects the development of DKD.This paper summarizes the role of ncRNA in the regulation of pyroptosis in DKD and the role of TCM in delaying the progression of DKD by regulating pyroptosis-related pathways,which provides research directions and ideas for the pathogenesis and clinical treatment of DKD.
