摘 要:目的:分析青少年肌阵挛癫痫(juvenile myoclonic epilepsy,JME)患者静息态功能磁共振成像(functional magnetic resonance imaging,fMRI)下默认网络(default mode network,DMN)的动态功能连接(dynamic functional connectivity,dFC)及其潜在的功能变化。方法:对37名JME患者和37名健康对照者(HC)的静息态f MRI数据进行组独立成分分析(group independent component analysis,GICA)。然后将所得独立成分与MNI模板匹配获得DMN中8个脑区。进一步采用滑动窗和K-means聚类构建该8个脑区的dFC,得到4个不同的dFC状态。结果:JME患者组与HC相比,左侧距状裂周围皮层与右侧距状裂周围皮层的dFC强度显著增强,左侧前扣带回与左侧内侧额上回、左侧前扣带回与右侧枕中回、左侧内侧额上回与右侧枕中回之间的dFC强度均显著降低。JME患者组相比HC在状态2时,停留时间(dwell time,Dt)更短,JME患者分数窗(fraction windows,Fw)显著减少。癫痫严重程度评分与Dt呈显著负相关。结论:JME患者DMN内双侧距状裂周围皮层的dFC强度增强可能与JME患者的光敏性相关,dFC分析可以为进一步揭示JME患者大脑的异常提供新的理解,并为后续研究提供参考。Objective:To analyze the dynamic functional connectivity(dFC)of default mode network(DMN)and its potential functional changes in resting functional magnetic resonance imaging(fMRI)of juvenile myoclonic epilepsy(JME)patients.Methods:The resting fMRI data of 37 JME patients and 37 normal volunteers were analyzed by group independent component analysis(GICA).Then,eight brain regions in DMN were obtained by matching the independent components with MNI template.Further,the sliding window and K-means clustering were used to construct the dFC of the eight brain regions,and the four different dFC states were obtained.The dFC intensity of JME patient group and healthy control group(HC)was compared.The significantly different indicators between the two groups were correlated with the national hospital epilepsy severity scale(NHS3)scores.Results:Compared with HC,the dFC intensity between the left Calcarine and the right Calcarine in the JME group was significantly enhanced,moreover the dFC intensity between left anterior cingulate and left medial superior frontal gyrus,left anterior cingulate and right middle occipital,left medial superior frontal gyrus and right middle occipital decreased significantly in the JME patient group.Compared with HC,in state 2,the dwell time(Dt)was shorter,and the fraction windows(Fw)was significantly reduced in the JME patient group.The score of NHS3 was significantly negatively correlated with the Dt.Conclusion:The enhancement of dFC intensity between left Calcarine and the right Calcarine in DMN of JME patients may be related to the photosensitivity of JME patients.The dFC analysis can provide a new understanding for further revealing the brain abnormalities of JME patients and provide a reference for subsequent research.
