补肾通络止痛方联合阿片类药物治疗骨转移癌痛30例  

Clinical effect of external application of Bushen Tongluo Zhitong prescription combined with opioids in treatment of bone metastatic cancer pain:An analysis of 30 cases

摘　　要：目的:观察补肾通络止痛方外用联合阿片类药物口服治疗骨转移癌痛的临床疗效。方法:将60例中重度骨转移癌痛患者随机分为治疗组和对照组,每组各30例。对照组根据三阶梯止痛法选择阿片类药物,治疗组在此基础上配合补肾通络止痛方外用,疗程均为7 d。比较2组数字评分法(NRS)评分、卡氏功能状态评分标准(KPS)评分、中医证候积分,以及不良反应发生情况。结果:总有效率治疗组为96.67%(29/30),优于对照组的86.67%(26/30),差异有统计学意义(P<0.05)。与治疗前比较,治疗后2组NRS评分、中医证候积分均减少,KPS评分均增加,且治疗组的改善幅度较对照组更明显,差异均有统计学意义(P<0.05或P<0.01)。不良反应情况比较,恶心呕吐的发生率治疗组为6.67%(2/30),对照组为26.67%(8/30),差异有统计学意义(P<0.05)。结论:补肾通络止痛方外用联合阿片类药物口服能明显缓解骨转移癌痛患者的疼痛程度,改善其生存质量,减少消化道不良反应的发生。Objective:To investigate the clinical effect of the external application of Bushen Tongluo Zhitong prescription combined with opioids in the treatment of bone metastatic cancer pain.Methods:A total of 60 patients with moderate-to-severe bone metastatic cancer pain were randomly divided into treatment group and control group,with 30 patients in each group.The patients in the control group received opioids selected based on the three ladders analgesia method,and those in the treatment group were given the external application of Bushen Tongluo Zhitong prescription in addition to the treatment in the control group;the course of treatment was 7 days for both groups.The two groups were compared in terms of numerical rating scale(NRS)score,Karnofsky Performance Scale(KPS)score,and TCM syndrome score,and safety was evaluated for the two groups.Results:The treatment group had a significantly better overall response rate than the control group[96.67%(29/30)vs 86.67%(26/30),P<0.05].After treatment,both groups had significant reductions in NRS score and TCM syndrome score and a significant increase in KPS score,and the treatment group had significantly greater improvements than the control group(P<0.05 or P<0.01).As for adverse reactions,there was a significant difference in the incidence rate of nausea and vomiting between the treatment group and the control group[6.67%(2/30)vs 26.67%(8/30),P<0.05].Conclusion:In the treatment of patients with bone metastatic cancer pain,the external application of Bushen Tongluo Zhitong prescription combined with opioids can significantly alleviate pain,improve quality of life,and reduce adverse gastrointestinal reactions.
